Please review all our rules and regulations before using the MBC facility.

The MBC staff may, from time to time, inform you about additional rules and regulations or changes.

All MBC members, guests and court rental players must check-in at the MBC front desk. Please have your club ID
card or identification document ready to show when checking in.

Anyone who does not sign an MBC liability release form will not have accessibility to MBC courts or facility.

Paying the guest fee enables the guest to enter and play in the MBC facility as a guest until he or she leaves the
MBC facility. There is no "complimentary re-entry."

MBC Management may limit the number of guests and court rental players that can use the MBC facility at any given
time.

MBC will accept personal checks for payment only from MBC members or if approved by administrative staff.

Children under 12 years of age MUST have Parental supervision at all times, on and off the courts.

MBC members and guests may not bring outside food or drinks into the Golden Gate Badminton Club. Only food or
drinks purchased at MBC Cafe or the MBC Pro Shop are permitted. Absolutely no food, colored, or sweet drinks are
allowed on the exposed Plastic floor court areas. Anyone who violates this policy will have to dispose the outside food
or drinks immediately or be required to leave MBC premises immediately without refund.

MBC members and guests are permitted to use the weight equipment inside the MBC facility without any lawful
liability to MBC. All members and guests use the weight equipment at their own risk. MBC has no responsibilities to
any physical injuries whatsoever incurred in the fitness room.

All members and Guests who use any of the MBC courts realize and accept that badminton is a vigorous sport and is
possible to sustain injuries when playing. All Members and Guests play badminton at their own risk.

Any MBC member who fails to pay past due fees or account payments for over 15 days will be receive a delinquent
notice suspending membership privileges until all past due fees or account payments have been paid. Any MBC
member who fails to pay past due fees or account payments for over 30 days will lose their membership status
without receiving any refund.

MBC does not allow smoking, gum chewing, animals or weapons in the facility. Anyone found to be violating this
policy will be required to leave the MBC premises immediately without refund and may be barred from entering the
MBC premises in the future.

Children must not, at any time, loiter or play in the parking area. If so, MBC will not be liable for any repercussions.

MBC is not responsible for any lost or stolen items. Please tend to your own personal effects.

MBC reserves all rights to refuse service and to revoke the membership status of any MBC member. Any requests for
refund will be assessed at the time. MBC reserves all rights to refuse any refund whatsoever.

In rare situations, a membership transfer will be granted upon request if a solid reason is provided and approved by
the MBC Directors Board. If transfer of membership is indeed granted by the MBC Directors Board, a processing fee
of $100 per applicant, and half of the membership period from the transferring member, will be granted to the
transferee. Membership transfer may only be granted once to each member. To assure proper processing,
application must be received before the 15th of any month and transferred membership will become effective on the
1st day of the next month.

Upon Membership due date, a 14 day grace period will be granted for renewal of membership. Once the 14 day
grace period expires without notification of renewal, membership will also expire. If expired members wish to renew
their membership after 14 grace period has expired, or renewal of membership beyond expiry date, a $25 processing
fee will be instigated for renewal of Membership application.

If court reservation is reserved by members, they will not have to pay drop in fees, only the court reservation fee, but
members friends must pay “regular” drop in fees, $12 which is to say, the cost of the drop in fee of the day, or at the
time of their entry into MBC to play. Please find out what is the cost of the drop in fee at the hour and day the court is
reserved. Each drop in fee by reserved court members guest once courts are reserved is a “one time” entry fee and
the guests are free to enjoy the facilities at MBC for the whole day providing they do not leave the premises for 1 hour
or more.

SUSPENDING MEMBERSHIP

You may suspend your membership for up to 2 months for life changing events such as Marriage, Birth of a child,
death in the family, and severe Illness or injury. A suspension fee of $15 per month, with an additional $15 processing
fee shall be implemented at the start of the suspension date. When suspending your membership, you must provide
proof of event in order to cancel and must be approved by the General Manager. If you do not return before the 2
month dead line, your membership will automatically expire and you will need to pay a $25 initiation fee to reactivate
your membership.

PRIVATE/GROUP LESSON POLICY

In CONSIDERATION of being permitted to participate in Badminton Activities at MIDWEST BADMINTON CLUB, I


and/or my minor child acknowledge and agree:

that MBC will to the best of its ability, make attempts to coordinate coaching with the coach of my/our preference. I
understand that should my coach of preference be no longer available due to uncontrollable events such as but not
limited to, illness, injury, death or termination, MBC will do its best to find a suitable replacement coach in a timely
fashion.

that the purchase of lessons is final and non-refundable.

that all lessons are to be completed within 6 months of the date of purchase.. Incomplete lessons following the 6
month period will be considered void and the value will not be refunded.

that credit for classes may be received for cancellation with at least 24 hours notice with approval of MBC
management. Notice must be confirmed by MBC staff. Cancellation within 24 hours or cancellation without notice
with void the scheduled lesson, and no credit will be given.

to the RULES AND REGULATIONS of the MIDWEST BADMINTON CLUB, and understand that failure to comply
with these rules may result in the forfeiture of any remaining lessons without refund.

STUDENTS & PARENTS: PLEASE READ THE FOLLOWING LESSON POLICIES:

Effective September 1st 2010.

Parents and students MUST be responsible to arrive at ALL lessons ON TIME

MUST BE AT LEAST a 50% commitment by Students for each months class in order to be able to attend our MBC
Training Classes

PAYMENTS MUST BE MADE ON OR BEFORE THE 1ST DAY OF THE CLASS OTHERWISE STUDENTS WILL
NOT BE able to attend class. If you are unable to pay on time, you must contact the General Manager.

We request Team members to be committed to classes because our Coaches are committed and our court times are
also committed. Students must diligently and seriously attend classes in order to achieve and improve.
During registration of monthly classes, all students MUST SPECIFY WHICH classes they will be attending for the
entire month. MBC is not responsible for unattended classes already purchased. Specification of dates, hours, and
times of classes are absolutely essential

All students/ parents MUST inform MBC within 24 hour written notice ( by e-mail to info@mbadminton.com) or verbal
notice during our office hours ( call 630 --------------- ), as a courtesy call, if they are not able to attend any of their
registered classes at anytime, as our courts have been committed to the lesson. MBC will not be responsible
whatsoever if not notified of unattended classes. There are no refunds or credit on any missed classes.

THERE WILL BE NO MAKE UP CLASS, REFUND, OR CREDIT FOR MISSED CLASSES. Parents and Students
are absolutely responsible to arrange students schedule to show up at the training classes 15 minutes before training
classes start.

For scheduled classes with 2 or fewer attending students. Classes will be reduced to 1 hours for Training Team, and
1 hour for Junior Team, as well as ALL age GROUP and Training teams.

TRAINING STUDENTS MAY NOT transfer classes. You must take classes where you paid for your classes ONLY.
Classes are NON transferrable regarding location, Coaches and Students.

**MBC reserves the right to vary terms and conditions of rules and policies without prior notice

Personal Conduct

MBC members, guests and court rental guests (herein referred to as "players") are subject to the control and
guidance of the MBC staff, and must wholly respect and follow staffs instructions and club regulations at all times
while playing in the club. Players agree to conduct themselves in a quiet and well-mannered fashion when in the club
and not cause any disturbances or interfere with the safe use or enjoyment of the club by other players. Players,
guests, or staff shall not conduct, purchase, or subscribe to any unauthorised commercial business, or activity, on
club premises or solicit other players nor business for any reason without the club's prior written or verbal approval.
Upon discovery of any unlawful solicitation whatsoever upon MBC premises, MBC shall reserve all rights to revoke
any membership or refuse entry of any and all players into the MBC facility. Under no circumstances shall players
interfere with the club's conduct of its business. Any complaints, other than those of an emergency nature or requiring
immediate attention by the MBC staff, shall be made directly to the MBC management in private. MBC does not
endorse the use of profanity/foul language due to the nature of any of its members. MBC is proud to maintain a family
environment and a high standard of enjoyment for all its members.

Dress Code

MBC reserves the right to deny entry to the club by any person whose attire or demeanor is not considered by the
MBC staff to be appropriate in connection with the public image of the club and its rules and regulations. No profane
language or slogans on any attire is permitted in the club.

Proper Athletic shoes and clothing are absolutely required in the court area for safety purposes. Absolutely NO
BLACK SOLE OR MARKING SHOES are permitted on the badminton courts. No jeans are allowed for playing in
MBC.

Responsible Use of the Club

Players should consult with a physician before using our services and facilities. Players understand and acknowledge
that MBC has no expertise in diagnosing, examining or treating any medical condition, including open cuts,
abrasions, sprains, sores, infections, maladies or inability to maintain personal hygiene, if such a condition poses a
direct threat to the health or safety of players and others. Players agree to use the facility in accordance with all
applicable public health requirements. It is the player's responsibility to consult with a physician to determine if any of
these conditions exists and, if so, whether such conditions pose a direct threat to the health and safety of the player
and others. MBC reserves the right, regardless, to make the final determination.
Waiver and Release

Player agrees that by engaging in any physical exercise, class or activity, or by using any club equipment or facility
on the premises, the player does so at his/her own risk. The player agrees that he/she is voluntarily participating in
activities and use of the facility and premises (including the parking lot) and assume all risk of injury, illness, damage
or loss to the player and player's property. The player agrees to release and discharge MBC, all affiliates, employees,
agents, representatives, successors and assigns from any and all claims or causes of action (known or unknown)
arising out of MBC's negligence. The player acknowledges that he/she has carefully read this Waiver and Release
and fully understand that it is a release of liability. Player is waiving any right that he/she may have to bring a legal
action to assert claim against MBC for its negligence.

Pro-Shop Warranty :

Due to the physically demanding nature of the sport of badminton, head breakage in racquets are common.
Unfortunately racquets manufacturers do not offer any warranty when breakage occurs in the racquet head area. In
some cases when breakage occurs in the handle or shaft MBC may ship the racquet back to the manufacture for
inspections for any material defects. MBC will not offer any further warranty other than what the manufacturer
provides.

Return Policy :

A MBC receipt must be submitted with the broken racquet for companies to consider replacing the racquet, if a
material defect is, in fact, determined. All other MBC pro shop merchandises and services are sold “as-is” without any
warranty. There are no refunds or returns on any products unless products are damaged and, if so, a receipt must be
produced in order to receive a full or partial refund. Damaged items must be returned for assessment within 7 days.
There will be no CASH refund. MBC will provide Pro Shop or MBC facility credit for all refunds.

Locker Rooms - Priority for Members Only

Daily, Bi Annual, or Annual rental of Lockers are available. Because of the limited number of lockers available, some
members may need to request their names be put on a waiting list for available lockers. Lockers will be provided first
come first serve. Numbered locks will be provided for a $10.00 charge but must remain property of MBC should you
relinquish your locker. Lock codes must be confidentially provided for MBC staff in cases of emergencies only.
Please do not leave valuables in the lockers. MBC is not responsible for lost or stolen articles. Lockers must be
paid in FULL for the category of days you chose. Only CASH payment upon rental of lockers will be accepted. Only
CASH payment of lockers will be accepted for each date payment is due. Members are responsible for
remembering their own due dates. Late payments will receive penalties of $5.00 per day beyond due date.

No perishable items may be stored in the lockers. Members are responsible for maintaining their lockers. Any
damage to the locker resulting from renter’s negligence shall be the responsibility of the member. A penalty of $200
will be implemented to member if lockers are damaged. MBC reserves the right to inspect the content of any locker at
any time.

If any member should give up their locker, it is their responsibility to completely remove ALL contents of their locker
upon expiry date of their locker. After 7 days, if all contents of locker are not completely removed, MBC reserves full
rights to clear out all the contents of the locker and dispose of it’s contents as MBC sees fit. MBC will not be liable
whatsoever for any contents left in an expired locker.

No hair dyeing, bleaching, or perming is permitted in the locker room. No glass containers are permitted.

Members and guests are responsible for maintaining sanitary practices in the Locker and washrooms at all times.
Please do not leave soap nor personal items in the washrooms. Please make sure all hair products or body products
are cleaned from face basins and shower stalls or toilets before you leave the washrooms. All lost articles are not the
responsibility of MBC and will be put in the lost and found bin if any articles are found in the washrooms or on MBC
facility.
