a) Provide copies of your HSE related policies
b) Provide HSE meeting planning for the current year
c) Provide minutes of 2 HSE meetings held last year and in the current year
d) Provide evidence of HSE communication - newsletters, posters, alerts etc

2 Planning
a) Provide your company's organizational chart showing HSE entity
b) Provide your organizational charter detailing HSE roles and responsibilities for management and other personnel
c) Provide a copy of HSE Objectives for last year and the current year
d) Provide your company's procedure for conducting risk assessments

3 Implementation
a) Provide HSE plan for the work scope in this tender
b) Provide risk assessment for this particular scope of work
c) Provide the risk management plan for the risks associated with this scope
d) Provide your company's guide for holding pre-job safety meetings
e) Provide agreement (letter) with your retainership clinic
f) Provide details (name, address, telephone etc) of your retainership clinic
g) Provide details of your company's policy for employee medical examination
h) Provide a copy of your company's PPE policy
i) Provide a copy of your HSE MS manual
j) Provide a copy of your Quality Management System manual
k) Provide evidence (3 reports) of service quality evaluations conducted
l) Provide your company's Emergency Response procedure
