1.0 INTRODUCTION

The preparation and submission of this report is a compliance of statutory

requirement of Occupational Safety and Health Act 1994. (Occupational Safety
and Health (Safety and Health Officer) Regulations 1997.

2.0 SCOPE

This report will cover the Safety & Health Officers report content as stipulated
under sub-regulation (2) of REGULATION 19 of Occupational Safety and Health
(Safety and Health Officer) Regulations 1997.

3.0 PURPOSE

This report is prepared for the purpose to acknowledge the employer pertaining
of HSE activities along the month.

4.0 HSE EXECUTIVE SUMMARY

❖ There is no accident or incident at site.

❖ Total Manhours MAR 2015 = 1536
❖ Total Manhours to date = 1728
❖ Contractor already registered with CIDB, registration No. 0320131128-
CH153755
❖ CIDB levi is paid on 24/01/14
❖ Toolbox meeting is conducted on weekly basis,

5.0 PROJECT SAFETY STATISTICS

5.1 Accident / Incident Report

• There are NO accident or incident occurrences at site for the month
of MAR 2015

5.2 HSE Promotion and Improvement

• Toolbox meeting

6.0 ACCIDENT INVESTIGATION AND REPORTING

• The HSE shall conduct an investigation within 24 hours where accident/incident

occurs at the site.
• The accident/incident shall be immediately reported to the Project Manager and HSE
Committee. The HSE Chairman shall convene an emergency meeting to discuss the
accident/incident and the immediate measures taken to correct and prevent a similar
accident/incident from further recurrence.
• The HSE Officer shall prepare and submit a monthly HSE report to CLIENT. The
report shall include but not limited to the following:

o Statistics
o Training
o Minutes of Meeting
o Safety Promotions

7.0 MONTHLY HEALTH, SAFETY AND ENVIRONMENTAL PROGRAMME

a) OSH Training / Course / Seminar / Exhibition

• Not recorded during this month

b) OSH Promotion and Improvement

• Safety Committee Chart already in placed but need to amend from

time to time& Safety meeting will be conducted once in every three
month.
• Toolbox Meeting is to be conducted on Weekly basis (Day and time
will be confirm)
• Job Safety Analysis (JSA) for every activity will be in place.
• Identification of Environmental aspect/impact and Safety & Health
hazard/risk assessment (HIRAC) will be in place.
• HSE penalty system to be proposed with an agreement with all
committee members.

c) Workplace HSE Inspection

• Inspection of the work area.

• Inspection of the machinery and tools.
• Inspection of proper PPE
