WAIVER, RLEASE AND QUITCLAIM

I, GIGI MADLANGAWA, Filipino, of legal age, single and a resident of 36 Times St., West
Triangle, Quezon City, after being sworn to in accordance with law, depose and state that:

1. I am the complainant in NLRC Case No. 7734 for ILLEGAL DISMISSAL entitled
MADLANGAWA vs. NACALABAN SERVICES, INC.;

2. I am an Employee of Nacalaban Services, Inc. hired on October 1, 2010 and assigned by the
latter as its Human Resource Assistant;

3. I have voluntarily resigned as HR Assistant of Nacalaban Services, Inc on January 13, 2018;

4. That I hereby acknowledge the receipt of One Hundred Thirty One Thousand Nine Hundred
Fifty-Six Pesos (P131,956.00) from Nacalaban Services, Inc. which is in full and final
satisfaction of all my claims that may be due me under the law;

5. That I hereby relieve and discharge Nacalaban Services, Inc. and its officers from any and all
claims by way of unpaid wages, separation pay, overtime pay, 13th month pay or otherwise as
may be due me or incident to my employment with said company;

6. That I hereby declare that I have no further claims whatsoever against Nacalaban Services,
Inc., its President, members of the Board, officers or any of its staff and that I hereby release
and forever discharge all of them from any and all claims, demands, cause of action of whatever
nature arising out of my employment;

7. That I am executing this instrument freely and voluntarily without any moral or physical threat,
coercion or promise of reward in any form.

IN WITNESS WHEREOF, I hereby affixed my signature this 21st day of April, 2018 at Quezon
City, Philippines.
