Students may redo a maximum of one (1) daily assignment per six week grade
reporting period and one (1) test per six week grade reporting period.
Students must have completed the initial assignment. (For example, simply writing
your name on the test or marking random multiple choice answers does not
constitute completing the initial assignment.)
Students must submit a completed Redo Request Form signed by the student and a
parent/guardian, coach, administrator or counselor for teacher approval. Redo
forms must be submitted no later than two days before the redo cutoff date for
each grade reporting period. Redo requests are granted at teacher discretion.
The higher grade (original or redo) will be the grade that is recorded.

Student Developed Plan of Action with evidence attached or tutoring/study log information indicated on
the back of this form (teacher’s initials indicate approval). Tutoring and study time must be arranged by
the student and must take place outside of instruction time.
