Reservation

 A Washington, DC sales tax of 10% and a gratuity of 20% will be automatically


applied to the final balance based on all food and beverage sales. Tax exemption is honored once
a copy of the tax exemption certificate is received.
 Final head count must be reported five (5) days prior to reservation date, otherwise the last number
given will be considered final.
 Cancellation of reservation within 24 hours of the reservation will result in a charge of $150. We
accept cash and major credit cards as forms of payment for the remaining balance.
 The “food and beverage” minimum is for food and beverage only and does not
include tax and gratuity. When applicable, a deposit may be required to hold the
reservation.
 Client will not be allowed to pay the final guest check using more than three credit cards.
 If the guaranteed food and beverage sales minimum is not met, the balance will be charged to the
credit card number listed below.
 The restaurant is not responsible for any articles of personal property – damages, lost, stolen, or
forgotten by guests.
 When applicable, guests will only be allowed to order food items from the pre-
selected menu chosen by the Client. If there are any changes to the Client’s pre-selected menu,
Ben’s Upstairs will contact the Client. Menu items are subject to change due to seasonal availability.
