C.

Accountability and Continuous Improvement

General Objective: A clear, transparent, inclusive, and responsive accountability system is in


place, collaboratively developed by community stakeholders, which monitors expected and
actual performance, continually addresses the gaps, and ensures a venue for feedback and
redress.

1. Roles and responsibilities of accountable person/s and collective body/ies are clearly
defined and agreed upon by community stakeholders.
Possible Documents:
a. Minutes of meeting with “Defining Roles and Responsibilities of Officers” as one of
the agenda.
b. Constitutions and Bylaws of school-based organizations.
c. Memorandums defining roles and responsibilities of chairpersons/coordinators
d. Appointment letters from the principal
e. Memorandums of Agreement (MOA) between the school and other community-
based organizations aimed at implementing projects/programs that will benefit the
students. (moa pagcor and handwashing facility) Immersion (MOA) Partnership
Proposals between ardci and JC Cua
2. Achievement of goals is recognized based on a collaboratively developed performance
accountability system; gaps are addressed through appropriate action.
Possible Documents:
a. Accomplishment/Year End Reports
b. Travel Itineraries
c. Financial statement
d. Transparency Board
e. Awards/ Congratulatory actions
f. Action research
g. Certificate of Recognitions both teachers and students
h. Tarpaulin

3. The accountability system that is owned by the community is continuously enhanced to


ensure that management structures and mechanisms are responsive to the emerging
learning needs and demands of the community.
a. Minutes of meeting stating that the school initiates enhancing community owned
accountability system
b. Letters of invitation or posters inviting community members and other stakeholders
to a dialog aimed to set up management processes, structures and mechanisms
c. Home Visits/School rules and regulations
d. Pictorials of community members doing their roles in educating the learners
e. Certificates of recognition garnered by the school’s best practices/programs /
projects/Products. (Rating Sheets used in evaluation should be attached)
4. Accountability assessment criteria and tools, feedback mechanisms, and information
collection and validation techniques and processes are inclusive and collaboratively
developed and agreed upon. (PROCESS)
Possible Documents:
a. IPCRF
b. Evaluation sheets/Rating Sheets of DepEd Projects/Programs implemented in the
school
c. Minutes of meetings with an agendum centering on the 4th objective
d. Include in the Agenda the lapses of the schools
e. Letter of invitation to community members to become evaluators in a school-based
programs.

5. Participatory assessment of performance is done regularly with the community.


Assessment results and lessons learned serve as basis for feedback, technical assistance,
recognition and plan adjustment.
a. Continuous Implementation documents (results, recommendations, actions
undertaken)
b. Quarterly exam signed by the parents
c. Special program culminating activities like the SPA Recital and the mounting of
outputs of SHS.
d. On the Job training documents of SHS
e. Minutes of meeting with the school’s performance assessment as one of the
agendum
