This permit relates to entry into any enclosed space and should be completed by the Master, Responsible Officer and by
all persons entering the space.
A. This permit fulfills the 'Risk Assessment' required for entering enclosed spaces. If any item is marked as 'NO', it is essential that
permission is obtained from the company and a safe system of work agreed prior making entry. Hazards as detailed in section
HSM 9.2.1.1 are also to be taken into account.
B. Validity of this permit should not exceed 8 hours and the maximum time the space can be left unattended shall not exceed 4
hours.
C. A separate checklist is to be filled for each compartment to be entered.
D. All previously issued 'Safety Permits' should be revoked immediately upon vessel's berthing and fresh permit should be issued
only after obtaining permission from port/ terminal.

6. Have arrangements been made for frequent atmosphere checks to be made while the space is occupied
and after work breaks?
(The interval will depend upon nature of enclosed space, duration of work and in any case shall not exceed 60 minutes)
7. Have arrangements been made for the space to be continuously ventilated throughout the period of
occupation and during work breaks?
8 Are access and illumination adequate?
9. Is a proper Fall Arrestor Device (FAD) available, checked and being used for tank entry unless permanent
fall prevention structure such as full length guard railings are provided on the vertical ladder ?
10. Are persons entering the space briefed about importance of maintaining 3-Point contact on ladders ?
11. Has an attendant(s) been designated to be in constant attendance at the entrance to the space?

SECTION 2 – PRE-ENTRY CHECKS (To be checked by Person InCharge of Entry Team and each person entering the space)
1. I have received instructions or permission from the Responsible Officer to enter the enclosed space
2. Section 1 of this permit has been satisfactorily completed by the Responsible Officer
3. I have agreed and understand the communication procedures
4. I have agreed upon a reporting interval of minutes
5. Emergency and evacuation procedures have been agreed and are understood
6. I am aware that the space must be vacated immediately in the event of ventilation failure or if atmosphere
tests show a change from agreed safe criteria

Notes:
1. For entry purposes, steady readings of all the following should be obtained: 20.9% oxygen by volume; Hydro Carbon content
not more than 1% of lower flammable limit (LFL); Toxic gases 0.0 ppm. TLV values to be checked from MSDS sheets or
Shore Office.
2. In order to obtain a representative cross-section of the space's atmosphere, samples should be taken from several levels and
through as many openings as possible. Ventilation should be stopped for about 10 minutes before the pre-entry atmosphere
tests are taken.
3. Tests for specific toxic contaminants, such as benzene or hydrogen sulphide, should be undertaken depending on the nature
of the previous contents of the space.
