exam must be marked "Employable" and signed by the company approved doctor.
Crew Operations
Dear Team Member,
Documents:
The passport, seaman's book (where applicable), original medical and crew data sheet (for returning
crew) will be collected when you board. Please have all documents ready when you leave the hotel. Do
not pack these documents in your suitcase.
Medical exam:
The medical must be completed prior to boarding by a CCL approved medical facility. It is your
responsibility to maintain a valid medical and have all the medical tests done as required by Carnival
Cruise Line. The medical should be valid for a minimum of 6 months from the sign on date for team
members in a tipped position or 8 months for those in a non tipped position. Please bring with you the
original medical forms, test results and proof of MMR Vaccinations to the ship. Pack medications (if any)
and your medical documents in your carry-on rather than checked luggage.
English Standard:
All new hires, with the exception of native English speakers, must carry a copy of their Marlins English
Language Test Score Sheet with them, and present it to the HR Department onboard.
U.S. Citizens must travel with a passport. A C1/D Visa is not necessary for United States Citizens, Alien
Resident Card (ARC) Holders and Canadian Citizens.
When applying for your visa, the application now requires the Ship's IMO number, please note below for
your reference.
Luggage:
We suggest you travel light due to certain luggage fees which airlines may levy on travelers. Carnival
will reimburse you for up to 2 bags upon boarding your vessel. Please present the HR Department
original receipts to be reimbursement. It is recommended that you carry some U.S. dollars to cover any
incidental or emergency costs.
Lost Luggage:
In the unfortunate event your luggage is lost when flying into the USA; please make a claim with the
airline and provide Carnival Cruise Line's address for delivery and include your crew ID number on your
luggage tags, so it can be sent to the ship you are scheduled to sign-on. Carnival Cruise Line address:

Upon boarding, please let the Manager Staff Administration (MSA) know of your lost luggage & provide
copies of baggage claim form/tracking number so he/she may assist you. If your luggage is lost when
you are flying home, please give your home address to the airline in your country of residence.
SMOKING IN HOTELS:
Most hotels are non-smoking and you will be responsible for any fine/penalty levied; please ask the front
desk staff for the designated smoking areas in your hotel.
Hotel stay:
Employees staying at these hotels at company's expense are ambassadors and representatives of
Carnival Cruise Line, therefore, your attire, grooming and personal hygiene affect both the public's
impression of the business and internal morale. This includes conducting yourself in a professional
manner whilst in public areas of the hotel such as the lobby, restaurant, gym and pool areas. Meals are
included with your hotel stay; see the hotel front desk attendant for the meal vouchers. Incidentals such
as movies & videos, phone calls, mini bar, internet etc., are not covered.
Airline tickets:
If you are flying to join your ship in another city, your airline ticket will be delivered to the hotel, check
with the front desk clerk. Be ready in the lobby, as indicated on the Final Assignment List in the Lobby
and the shuttle bus will take you to the port or airport. If your name is not on the list, call the Shipboard
Human Resources department immediately at (305) 406-4649 or 1-888-871-7150.
Arrival Confirmation:
You must confirm your arrival prior to the scheduled arrival date. You will receive an automated email
asking you to confirm your arrival at least 21 days before your scheduled arrival date. You may confirm
using this email link. If you do not confirm your arrival, you may lose your ship assignment, and will be
required to pay any associated hotel costs.
Manning Agency: (applicable to crew from Indonesia and Philippines, Malaysia, China):
All returning crew must contact their respective Manning Agency within one week after your arrival at
home. This applies to crew from Indonesia and the Philippines.
IMPORTANT ARRIVAL CONFIRMATION INFORMATION
Even though the ticket may be provided to you by Carnival, you still need to confirm your arrival. If you
report to your agent they will send the confirmation on your behalf. Please make sure you report to your
agent within 7 days of being home. If you are rescheduled, you need to confirm your arrival again.
Please check your spam folder for the auto email, should you not have received it. In which case, you
may write to crewassistance@carnival.com with the following information.
Non-Confirmation:
If no arrival confirmation is received within 4 days of your arrival date, the ticket and your assignment will
be cancelled.
No shows:
If you do not advise us about not returning prior to your travel date, it will be regarded as a no show and
you will be responsible for your own ticket if we choose to reschedule you in the future.
Schedule changes:
Requesting changes to your joining date should be restricted to emergency situations and there may be
a cancellation fee for the ticket which you will be responsible for.
Physical exam information for Crew
The following questions and answers are designed to assist you with the completing the Carnival Cruise
Line's physical exam process that is required before you begin sea duty:
What if the medical certificate is due to expire before the end of my scheduled contract?
Medical certificates must be valid for the duration of the crew member's scheduled contract. If a crew
member presents a medical certificate due to expire before the end of his/her scheduled contract, the
crew member may not be allowed to join the ship. In such cases, the crew member will assume all costs
of returning home.
If I do not pass the exam, do I still have to pay for it? Yes.
Do I need to bring a copy of the physical exam documents with me to the ship?
No, copies are not accepted. All original medical forms will be collected from you upon boarding the
vessel. You may not be allowed to sign- on the ship unless you are in possession of them.

Please be advised that the team member below is confirmed to join the Carnival Legend in
Sydney,Australia on December 6th, 2016.
We kindly request the necessary visa to be issued to the above mentioned team member in order to
sign on the ship and he/she will be departing Australia on the vessel.
Our Carnival Australia office, As agent for the vessel's owners,located at L5, 15 Mount St, North Sydney,
NSW 2060, will assume full responsibility for the maintenance of this team member whilst in Australia
and repatriation to their home country in the venet of failure to join the ship for any reason . Overnight
accomodation, if required, will be provided as well as airport to hotel and hotel to ship transfers.
