Introduction

Introduction
介 紹

The Oxford English Dictionary definition of “Standards” is “Something which is generally recognized
as being correct or acceptable”.

Logic and common sense tells us that a large proportion of accidents related to a relatively small
amount of construction activities could be minimized by compliance with established relevant physical
safety standards. Such standards serve as effective control measures to minimize or eliminate the risk
of accidents whilst carrying out such routine construction activities.

When talking about Physical Safety Standards we refer to the minimum control measures, features or
requirements, which should be established and maintained throughout all aspects of the physical
workplace.

In order to both clarify, and establish consistent physical safety standards across our business,
Gammon HSE Dept have produced a “Book of Safety Standards” which, will be the basis for the
minimum safety controls, features and requirements for all aspects of the workplace.

The intention of the “Book of Safety Standards” is not for it to act as a replacement to written
procedures or established acceptable work practices but to act as additional guidelines for individuals
engaged in construction activities.

The “Book of Safety Standards” should be studied and fully understood by all Line Managers and both
issued & communicated to all applicable employees.

Full compliance with the requirements of this “Book of Safety Standards” will be expected on all
Gammon sites.

Requirements : R1. The design of electrical systems (both temporary and permanent) should meet the
(# - Statutory) statutory requirements. #
R2. All electrical switchboards and equipment should be installed, maintained and
regularly checked by a Registered Electrician. #
R3. No work shall be done on live conductors or equipment.
R4. All switchboards and equipment shall be properly earthed. Their external metal
casing shall also be properly earthed, except for double insulated equipment. #
R5. The distribution boards and switchboards shall be locked with contact details of key
holder provided.
R6. Coloured marking for circuits and appliances shall be clearly distinguished. #
R7. The distribution circuits shall be equipped with suitable protection devices such as Main
Circuit Breaker (MCB) and Residual Current Device (RCD) to protect against
over-current and earth leakage respectively. #
R8. All wiring shall be supported on proper insulators and the sheath of all electric portable
cables shall be of heavy-duty. #
R9. All flexible cables shall be protected against kinking and be relieved from mechanical
strain at connections to terminals. They shall be hung up as far as practicable.
R10. A permit-to-work certificate (OCP-hsms-13 Form 1) shall be obtained before
maintenance works commence or connections are made to the switchboard.
R11. Portable and hand held tools shall be from voltage of 110v or less and equipped with
a built-in switch for when the tool is released from the hands.

Safety : S1. All switchboards shall be free from obstructions. #


Precautions S2. Control appliances such as switches, fuses and circuit breakers shall not be installed
at places near flammable liquids or flammable gases.
S3. All electrical equipment shall be inspected before use and suitable for its proposed
use.

Requirements : R1. Worker who works near electricity supply lines shall be informed the hazards and
(# - Statutory) trained to follow the permit-to-dig procedures. #
R2. A competent person (approved by EMSD) shall be appointed to locate the alignment
of all U/G cables in the area, irrespective of the excavation depth. #
R3. The competent person shall be on site at all times to supervise the works during the
trial pit excavation. #
R4. U/G cable alignments and depth profiles as identified in the area shall be clearly
marked on the ground. #
R5. The written report by the competent person, which contains information of the U/G
cable’s alignments and depth as identified, shall be obtained and kept. #
R6. Portable power tools shall only be used for breaking out paved concrete surfaces or
adjacent to the cable. #

Safety : S1. Always maintain co-ordination with electricity suppliers to obtain further guidelines as
Precautions necessary for works in the vicinity of U/G cables.
S2. U/G cables found in roads, footpaths and on sites, should always be treated as LIVE.

Requirements : R1. A competent person shall identify whether the proposed works within the vicinity of
(# - Statutory) O/H lines are necessary (i.e. a horizontal distance of 9m from the nearest conductor
for all works except blasting work, for which the horizontal distance is 25m). #
R2. Worker who are involved in works near O/H lines, especially the operators of lifting
and earth moving equipment, shall be informed the hazards and trained to follow the
working procedures.
R3. The electricity source shall be safely isolated before and during the course of work. #
R4. Ascertain the voltage rating, distance to ground of the O/H lines and consult the
electricity supplier of the lines to agree on a mutually acceptable working plan if any
of the proposed works fall within the vicinity of the O/H lines.

Safety : S1. Contact the electricity supplier to arrange a site meeting before starting works.
Precautions S2. Avoid unnecessary working underneath the O/H lines.
S3. Never rely on the insulating cover of any overhead electricity conductors as means of
protection from electric shock.
S4. Always treat the insulated conductors as LIVE.
S5. Do not erect structures (e.g. scaffolding) unnecessarily in the vicinity of O/H lines.

Features : F1. A safe working plan, which contains headroom, voltage, safe working distance and
safe working procedures.

Requirements : R1. A Registered Electrician shall carry out all electrical installations. #
(# - Statutory) R2. The welding equipment shall be protected by a separate switch on a separate circuit
that isolates the welding set from other equipment.
R3. Arc welding machines shall be controlled by a switch mounted on or near the
machine framework that, when opened, immediately cuts off the power from all
conductors supplying the machine. #
R4. Arc welding set shall be installed with electrical shock preventor / voltage reducing unit.
#
R5. The residue current device (RCD) shall be installed at the power source, if there is any,
to give protection against electric shock that occurs at the input side of the welding
transformer.
R6. Frames of arc-welding machines shall be effectively earthed. #
R7. Return conductors shall be taken directly to the work and securely connected
mechanically and electrically to it or to the workbench, floor, etc. and to an adjacent
metallic object. #
R8. Electrodes shall only be inserted in the holder with insulating gloves.
R9. The outer surface of electrode holders of hand-operated arc-welding machines,
including the jaw shall be effectively insulated. #
R10. Appropriate PPE shall be used. Refer to the requirement of PPE of this Book of
Safety Standards.

Safety : S1. Welding circuits shall be switched off when not in use.
Precautions S2. Electrode conductors or cables shall not be excessive in length.
S3. Forced ventilation shall be provided for welding operation where required.

Requirements : R1. Persons who might fall more than 2m shall be protected by means of adequate catch
(# - Statutory) nets, sheets or platforms, or be secured by safety harnesses with the independent
lifeline properly attached. #
R2. Working platforms shall be protected on all sides with top guardrails (900mm to
1150mm high), intermediate guardrails (450mm to 600mm high) and toe-boards (at
least 200mm high). #
R3. Working platforms shall be provided with safe means of access and egress, which
are at least 400mm wide for passage of persons only, or at least 650mm wide for
passage of materials, and of sufficient length. #
R4. Only trained workers with adequate experience and under the supervision of a
competent person shall erect, alter or dismantle handrails and toe-boards. #
R5. Working platforms and scaffolds shall be regularly inspected. Scaffolding working
platforms shall be periodically (14-days) inspected and displayed with Form 5 by the
competent person. #
R6. Harness, lanyard or independent lifeline shall not be anchored to any railings or
fittings of working platform, unless they have been specified and tested for the
purpose.

Safety : S1. Working platforms shall be good design and construction, adequate strength and
Precautions made of sound material and free from patent defect. #
S2. Working platforms shall be kept clean and free of projecting features.
S3. Sufficient numbers of stanchions or standard poles or uprights shall be maintained to
ensure sufficient stability and resistance. #

WORKING AT HEIGHT PROTECTION OF FLOOR OPENINGS / WALL OPENINGS

Construction Sites (Safety) Regulations
Code of Practices for Personal Protective Equipment

Requirements : R1. Floor opening through which person could fall shall be guarded by covers or by
(# - Statutory) guardrails and toe-boards on all exposed sides. #
R2. Wall openings less than 1m from the floor and measuring at least 750mm vertically
and 450mm horizontally from which there is a drop of more than 2m shall be
protected by guardrails and toe-bards. #
R3. Narrow wall openings shall be protected by a toe-board is their lower edge is less
than 200mm from the floor. #
R4. The covers for floor opening shall be secured to allow walk on safely and if
necessary, to drive on safely. #
R5. The guardrails and toe-boards as required in R1/R2/R3 shall comply with the
requirement of General section. #
R6. If the means of protection is removed to allow the passage of persons or goods or for
other purposes, it shall be resumed as soon as practicable.

Safety : S1. Covers for openings shall not constitute any obstacle to persons and traffic.
Precautions S2. No debris like concrete waste placed or stored onto the openings is allowed.
S3. No shock loading on the guardrails or toe-boards is allowed.

Features : F1. Covers or guardrails and toe-boards with mesh to prevent falling objects. #

Construction Sites (Safety) Regulations

Requirements : R1. Ladder shall only be used for simple work of short duration.
(# - Statutory) R2. Ladder shall not be used as a working platform.
R3. Ladder shall be erected on a firm and level base and at a base angle exceeding 75
degrees or a 4 (rise): 1 (base) ratio. #
R4. Ladder shall be secured at top. When it is impracticable to do so, the ladder shall
also be firmly secured at base.
R5. Ladder shall be at least 1.05m above their landing place or above the highest rung
for handhold. #
R6. The spreaders of a stepladder shall be locked in position.
R7. A landing point for rest purpose shall be provided every 6-m for vertical cat-ladder. #
R8. Climbing ladder over 3m shall install back loop. #
R9. Podium ladder shall be provided with guardrails and toe-board as required in
General section.
R10. Wooden ladder shall not be permitted. For electrical work, the footing of ladder shall
be fully insulated. Special cases, which require wooden ladder, e.g. an electrician
working in energize room, shall be approved by the SHS&ER or his delegate.

Safety : S1. Ladder shall be of good construction with no missing or defective rung, of sound
Precautions material and of adequate strength for its intended use.
S2. Ladder shall be checked before use.
S3. It shall not step on the top two rungs of an A-shape ladder.
S4. Workers who might fall more than 2m from the ladder shall use safety harnesses as
required in General section.

Code of Practices for Personal Protective Equipment

Requirements : R1. Safety nets shall be erected if it is impracticable to provide adequate guardrails or
(# - Statutory) platform access conforming to the requirement listed in General section. If the
erection of safety net is also impracticable, safety harnesses shall be used to prevent
injury due to fall of persons.
R2. Safety net shall be installed with sufficient clearance under them to prevent contact
with the surface or structures below when subjected to an impact force equal to the
drop test.
R3. Sufficient anchorage fittings shall be provided for safety harnesses with lanyard
attached to independent lifeline. #
R4. Safety net shall be inspected at least once a week for wear, damage and other
deterioration.
R5. Safety harnesses, lifeline, anchorage and fittings shall be properly maintained

Safety : S1. Safety net shall extend outward from the outer most projection of the work surface,

S2. When safety net is used on bridges, the potential fall area from the walkway or

Requirements : R1. Certified workers and competent persons should complete an approved training
(# - Statutory) course. #
R2. A risk assessment with recommendations should be carried out by a competent
person and updated due to any subsequent changes. #
R3. Permit-to-work certificate stating all necessary safety precautions should be strictly
followed and fully communicated with the involved workers. #
R4. A certified worker should be stationed outside the confined space to maintain
communication with the workers inside. #
R5. Personal protective equipment as recommended within the risk assessment report
should be strictly used. #
R6. Where the use of approved breathing apparatus is recommended in the risk
assessment report, a worker entering or remaining in a confined space should be
wearing a suitable safety harness connected to a lifeline that is strong enough to
enable him to be pulled out, and that the free end is held by a person outside the
confined space who has sufficient physical strength to be capable of pulling the
person out of the confined space. Where practicable, appropriate lifting equipment
should be available for rescue purposes. #

Safety : S1. No worker should enter a confined space before a risk assessment has been carried

S2. All safety measures recommended in the risk assessment report should be fully
implemented.
S3. The person in-charge of the Permit-to-work certificate should ensure all workers,
materials and equipment involved in the confined space work are withdrawn when

Requirements : R1. Shall maintain every workplace with adequate ventilation by means of circulation of
(# - Statutory) fresh air (via natural or mechanical ventilation) and free of dust, fumes or other
impurities. #
R2. If dust or fumes or other impurities are given off at the workplace, all practicable
steps shall be taken to protect workers against inhalation of the dust or fumes or
other impurities and to prevent accumulation of such dust or fumes. #
R3. Exhaust appliances shall be provided and maintained as near as possible to the
point of origin of the fumes, dust or other impurities, e.g.

R6. The ventilation system shall be inspected, maintained and cleaned regularly by
competent person. #
R7. Effective antiseptic measures shall be implemented to prevent the breeding of
biological contaminants (e.g. mosquitoes) in all site area. #
R8. Personal protective equipment shall be used in cases where dust, fumes and/or
other impurities are present in the workplace. #
R9. An appointed competent person shall perform gas detection as necessary (regularly
or continuously subject to risk assessment). The result of gas detection shall be
displayed in a highly visible location at the workplace. #

Requirements : R1. Site management should conduct a basic hazard assessment on the pedestrian /
(# - Statutory) vehicle control within the site and document it as a traffic management plan. #
R2. Where practicable, all walkways should be clearly demarcated with cones or
barriers.
R3. The standard width of walkways is 1m and the minimal width is 750 mm.
R4. Demarcation of storage areas, and the forbidden areas to be left clear (e.g. floor area
below electrical panels and safety equipment), is recommended.
R5. Roadways for vehicles should be wide enough to accommodate any vehicle
authorized to be used in that area. Traffic barriers and signs should be placed where
required.
R6. All workers exposed to the risks of moving roadway traffic or construction plant
should wear high-visibility garment. #
R7. The exits and entrances to public roadways / walkways should be well planed to
minimize potential hazards and any inconvenience of both public and workers.
Warning signs / lights for both daytime and nighttime work shall be used.
R8. A traffic banksman shall be stationed where required, especially for main exits and
entrances.

Requirements : R1. Site management should document a Temporary Traffic Diversion Scheme after
(# - Statutory) consulting the appropriate authorities (e.g. Highways Department, Transport
Department, the Police and KCRC). Where applicable, Excavation Permit and
Expressway Work Permit should be obtained and displayed on site entrance. #
R2. Setting up the works area should commence with the placing of traffic control
equipment (e.g. traffic signs, cones, barriers, lanterns and road marks) in
accordance with the Temporary Traffic Diversion Scheme and the Code of Practice
for the Lighting, Signing and Guarding of Road Works. #
R3. When the signing layout has been established, it is advisable to drive/walk through
the site to check the signing from the driver/pedestrian point of view.
R4. During the hours of darkness, all obstruction and barriers should be properly lit with
road hazard warning lanterns. Adequate lighting should be provided in case of work
at night. #
R5. Pedestrians should be protected from the works by means of continuous pedestrian
barriers. #
R6. Vehicles engaging in moving works should be conspicuously painted bright yellow in
colour. If required in the Temporary Traffic Diversion Scheme, they shall also be
fitted with amber flashing roof beacons and alternate red and yellow fluorescent and
reflectorised markings. #
R7. All workers should wear high-visibility garment. #
R8. The adjacent carriageway and footways should be kept free of loose stones, material
and debris.
R9. Remove all the signs, barriers, and materials when the work is completed. #

Safety : S1. All traffic control equipment should be stable under adverse weather conditions and

Requirements : R1. Operators, banksmen and competent persons of lifting appliances shall hold valid

R2. A competent person shall be appointed to supervise lifting operation. #
R3. Lifting appliances shall be tested and examined by competent examiners prior to
use. #
R4. Lifting appliances shall comply with the requirements in Plant and Equipment of this
Book of Safety Standards.
R5. An Automatic Safe Load Indicator (ASLI) shall be installed. #
R6. Cabins of lifting appliance shall not be covered to provide clear and unrestricted
view. #
R7. Safe access shall be provided to all parts of machinery for periodic inspection or
maintenance. #
R8. Before lifting operation, the load’s characteristics (size, weight, any anchorage point,
stability, etc) and floor condition shall be considered to determine the type of crane,
lifting gear and method of lifting. #
R9. Load shall be safely secured before lifting and maintained in stable condition and
free from swinging.
R10. Permit-to-Operate (PTO) and Permit-to-Move (PTM) shall be obtained and displayed
on the plant before operation and moving.
R11. A designated banksman is required if the operator’s view is blocked and signaling is
required. #

Safety : S1. No crane shall be used under weather conditions likely to endanger its stability.
Precautions S2. Never overload the lifting appliance.
S3. No single lifting appliance or lifting gear shall be loaded beyond its safe working load
or rendered unstable in the raising or lowering of the load.
S4. Special risk assessment and method statement shall be developed for tandem lifting
operation, lifting operation at slope, or traveling with loads suspended.
S5. Never disable the safety catch.

Requirements : R1. The operation area shall be barricaded.

the float and completely support the float.

permanent, i.e., switches locked off, fuel supplies cut off and any doors giving access
to machinery or control cabins locked to prevent unauthorized access. The ignition
key and any other keys should be removed from the crane at any time when the
operator is absent from the machine.

LIFTING OPERATION TOWER CRANE

Requirements : R1. Mounting base shall be designed by a professional engineer and checked by ICE.
(# - Statutory) R2. Crane operators shall conduct regular medical check to ensure they are agile, have
good eyesight and are not height phobic.
R3. Effective earth connections shall be provided for electrically-operated cranes.
R4. Loading shall be lifted vertically and not towed.
R5. Tandem lifting by tower crane shall be strictly prohibited.
R6. In case of more that one tower crane was installed in the same site, the operation
overlap zone shall be identified and an anti-collision system shall be set up after
consulting Plant & Equipment Department of CSD.
R7. A boundary limited device shall be installed after consulting Plant & Equipment
Department of CSD to prevent any load from moving outside from the site boundary.
R8. When adverse weather conditions are expected, the main jib should be slewed to the
side of the tower away from the wind and put into free slew; the power supply
switched off and locked; any fuel supplies cut off; and doors or panels giving access
to electrical or mechanical equipment closed and locked.
R9. For longer periods and for out-of-service conditions, isolation should be more
permanent, i.e., switches locked off, fuel supplies cut off and any doors giving access
to machinery or control cabins locked to prevent unauthorized access. The ignition
key and any other keys should be removed from the crane at any time when the
operator is absent from the machine.

Safety : S1. Before being left for a long period, tower cranes which do not have continuous

Lifting Operation

LIFTING OPERATION EXCAVATOR FOR LIFTING OPERATION

Requirements : R1. An excavator used for lifting operation shall be designed suitable for the purpose.

Requirements : R1. Lifting gear shall be tested and examined by competent examiner prior to use.

R2. The safe working load shall be clearly and legibly marked on lifting gear, in English
and Chinese, together with an identification mark. #
R3. Colour code system shown as Photo 4 shall be adopted.
R4. Lifting gear shall be properly stored under cover in clean, dry, well-ventilated places
to prevent corrosion or other damage.
R5. Invalid / damaged lifting gear shall be painted in black and removed off site or cut to
prevent illegal use.
R6. Where necessary to prevent sharp bends in slings, corners of loads shall be
adequately padded. #
R7. The load shall be maintained balanced and stabilized.
R8. The lifting process shall be maintained smooth and slowly.
R9. When handling composite loads such as loose bundles of tubes, bars or wooden
battens, such composite loads shall first be tied up securely at their ends by steel
wires or similar means of adequate strength.

Requirements : R1. Permit-to-work for Excavation shall be obtained before excavation begins on site. #

Requirements : R1. The operator shall be 18 years old or above, hold a valid certificate and be
(# - Statutory) competent. #
R2. Tower crane shall be tested and thoroughly examined by a competent examiner after
erection or every 12 months with Form 2, 3 & 5 filled up. #
R3. Tower crane shall be inspected by a competent person every 7 days with Form 1
filled up. #
R4. A suitable cabin shall be provided to protect the operator from the weather and give
him a clear and unrestrained view. The cabin shall be fitted with a lock to prevent
unauthorized entry and fire extinguishers. #
R5. Tower crane shall be fitted with an automatic safe load indicator (ASLI). This does
not apply to a crane with a maximum safe working load of 1 tonne or less. #
R6. Every brake on the tower crane shall be fail-safe, i.e. the brake will be automatically
applied wherever there is loss of power. #
R7. Tower crane shall be marked with identification and the safe working load (SWL), in
English and Chinese. #
R8. Tower crane shall be equipped with built-in safety devices, i.e. a hook height limit
switch, luffing jib limit switches, a trolley travel limit switch, etc. which operate
automatically to prevent damage to the machine should the operator make an error.
#
R9. Safe mean of access and egress (e.g. access ladders with safety hoops or
guardrails) shall be provided. Suitable rest platforms shall also be provided at
intervals of not greater than 9m. #
R10. All exposed moving parts of a tower crane such as gears, pulleys, belts, chains,
shafts, flywheels, etc. which might constitute a hazard under normal operation
conditions shall be effectively guarded. #
R11. Electrically-operated cranes should have an effective earth connection and all tower
cranes should be completely and correctly earthed for lightning protection. #
R12. Proper communication channel should be established between operator and slinger /
signaler, e.g. radio and walkie-talkie. #
R13. In case of more that one tower crane was installed in the same site, the operation
overlap zone shall be identified and an anti-collision system shall be set up after
consulting Plant & Equipment Department of CSD. #
R14. A boundary limited device shall be installed after consulting Plant & Equipment

R3. Builders’ lift shall be tested and thoroughly examined by a competent examiner after
erection or every 6 months with Form 14, 15 & 16 filled up. #
R4. Builders’ lift shall be maintained, inspected, cleaned, oiled and adjusted by a
registered contractor at least every 7 days with Form 7 filled up. #
R5. The most recent certificate of test and examination shall be prominently displayed
inside the lift cage or on the platform. #
R6. Builders’ lift shall be checked and inspected by a competent operator daily for its
function before commencement of operation. The checks shall at least include the
tyre pressure, hydraulic oil level, operation on the controls, hydraulic oil leakage, hose
and connections, chassis mountings and linkages. #
R7. The number of passengers in the lift cage shall not exceed the maximum number
permitted. #
R8. No safety devices shall be interfered while operating the builder's lift. #
R9. Proper operating procedures laid down by the manufacturer shall be followed while
operating the builder's lift. #

Safety : S1. The design and construction of builders’ lift shall be complied with the Code of
Precautions Practice on the Builders’ Lift and the following requirements: -
a. A builders’ lift shall be of good design and construction, adequate mechanical
strength, and properly installed and anchored. #
b. A builders’ lift shall be provided by a cage which is constructed such that persons
shall not fall out of the cage or be trapped between any fixed structure or moving
parts of the lift or be struck by articles or materials falling down the lift. #
c. The cage shall be fitted with a gate which has efficient interlocking to prevent the
cage from being opened unless the cage is at a landing place and to prevent the
cage from moved away from the landing place until the gate is closed. #
d. The landing place in a lift enclosure shall be fitted with a gate which is fitted with
efficient interlocking or similar device. #
e. The lift cage shall be fitted with efficient automatic devices to cause the cage to

come to rest at a point above the lowest point to which it is able to travel. #
f. The lift cage shall be fitted with an overload sensing device to cause the cage to

lift cage or as a trapdoor in the lift cage roof and a safety switch to interrupt the
control circuit and prevent the movement of the lift cage if the emergency
opening is not properly closed or locked. #
h. The lift shall be fitted with effective safety gear and over speed governor to cause
the cage to come to rest when maximum speed exceeded. #
i. The lift shall be fitted with an effective driving machine brake. #
S2. The following markings shall be clearly shown on the builders’ lifts: -
a. The safe working load; the maximum number of persons it may carry; the
maximum wind speed exceeding which no operation shall be permitted and the

Requirements : R1. All abrasive wheels shall be properly mounted by competent persons who have been
(# - Statutory) appointed in writing by the employer. They shall be well trained and have the
practical experience for the mounting job. #
R2. Every wheel shall be clearly marked, in English or Chinese, with the maximum
permissible speed in revolutions per minute specified by the manufacturer of that
wheel. #
R3. No abrasive wheel shall be operated at a speed in excess of the maximum
permissible speed. #
R4. No spindle shall be operated at a speed in excess of the lesser of the maximum
working speed of that spindle and, while an abrasive wheel is mounted on it, the
maximum permissible speed of the wheel. #
R5. A guard shall be provided and kept in position at every abrasive wheel in motion.
Suitable protection flanges shall be mounted if necessary. #
R6. An efficient device for starting and cutting off the power shall be provided to the
machine where abrasive wheels are used. #
R7. Where at an abrasive wheel there is a rest for supporting a workpiece, that rest shall
at all times while the wheel is in motion be properly secured and adjusted so as to be
as close as practicable to the exposed part of the abrasive wheel. Every rest shall be

arising from the use of abrasive wheels and the precautions to be observed shall be

R9. The floor immediately surrounding every fixed machine on which an abrasive wheel

R10. Persons carrying out dry grinding operations and truing or dressing an abrasive

Requirements : R1. The driver of loadshifting machine shall be 18 years old or above and hold a valid
(# - Statutory) certificate. #
R2. The driver of truck shall have a valid driver license for the specific truck. #
R3. The dumper and truck shall not be overloaded or over speeded. #
R4. Mirrors, reverse alarm and warning alarm shall be provided. #
R5. Temporary fencing and warnings at edges of embankments, excavations and pits
shall be provided. Safety measures for dumping pits such as stop logs to rear wheels
of dumpers shall be provided. #
R6. The tipping bodies shall be lowered and the engine shall be switched off in an
unattended vehicle or machine. The machine shall be parked on level ground, in
neutral with the parking brake applied.
R7. Where required, bandsmen shall be designated for assisting in the dumping
operations. They shall be provided with PPE (safety helmet, reflective vest and
safety boots, refer to the requirement of PPE in this Book of Safety Standards) and
communication means.
R8. The dumper and truck driver shall be provided with PPE (safety helmet, reflective
vest and safety boots) when getting off the driver cabin on site. #

Safety : S1. Be aware of the overhead power lines and cables.


Precautions S2. The machine shall be provided with periodic servicing according to the
manufacturers’ requirements. #
S3. Any loose parts, damage or oil strain shall be reported to the site supervisor who will
then arrange repair.
S4. No passenger shall be carried on a dumper except the driver. #

Requirements : R1. The operator shall be 18 years old or above, hold a valid certificate and be
(# - Statutory) competent. #
R2. Excavator shall be tested and thoroughly examined by a competent examiner after
erection or every 12 months with Form 3 & 5 filled up. #
R3. Excavator shall be inspected by a competent person every 7 days with Form 1 filled
up. #
R4. Excavator shall be provided with seat belts and a falling object protective structure
(FOPS), roll over protective structure (ROPS) or tip over protective structure (TOPS).
#
R5. A permit-to-dig shall be obtained before commencing works. #
R6. Mirrors, warning flashlight, mega phone, audio sensors, and protection bars with
flexible hinges, shall be provided. #
R7. The work area of excavator shall be fenced off on all sides as far as practicable. An
unobstructed passageway of not less than 600 mm shall be maintained between the
slewing or other moving parts of the excavator and any fixed fixtures. Warning signs,
in English and Chinese, shall be displayed at the back of excavator #
R8. Excavator shall be rested on firm ground during operation and parking.
R9. Excavator shall be parked with parking brake firmly applied and blocking wedges
used where appropriate. The bucket or other type of working tools shall be lowered
to ground surface.
R10. Bandsmen shall be designated when reversing or in case the operator's view is
restricted. They shall be provided with PPE (safety helmet, reflective vest and safety
boots, refer to the requirement of PPE in this Book of Safety Standards) and
communication means. #
R11. The operator shall be provided with PPE (safety helmet, reflective vest and safety
boots) when getting off the driver cabin on site. #

Safety : S1. Refer to the requirement of Excavation and Electricity of this Book of Safety
Precautions Standards for the precautions of underground utilities and overhead cables.
S2. Excavator shall be chosen considering its capacity, intended purposes and site

conditions.
S3. Unauthorised movement or use of the excavators shall be prevented at all times.
S4. Excavators shall be regularly maintained with records kept on site. #
S5. Operation and maintenance manuals, manufacturer’ s specifications, inspection and
maintenance log books shall be provided where required. #
S6. It should avoid operating the machine too close to an overhang, deep ditch or hole
and be alert to potential carving edges, falling rocks and slides, rough terrain and
obstacles.

Requirements : R1. Gas welder and flame cutter shall be 18 years old or above, hold a valid certificate
(# - Statutory) and be competent. #
R2. A Dangerous Goods Store shall be provided if storage quantity exceeded exemption
quantity. Refer to the requirement of Fire Protection of this Book of Safety Standards.
R3. Flash back arrestors and non-return valves shall be provided for both oxygen and
acetylene cylinders. #
R4. Cylinders shall be held in an upright position by straps, collars or chains.
R5. Cylinders shall not be knocked, dropped or rolled in handling, or otherwise subjected
to violent shocks.
R6. Cylinders shall be locked in cage or in trolley, and stored separately for acetylene
and oxygen cylinders, or empty and charged cylinders, with proper signs and label.
R7. Trolley or lifting cages shall be used for transporting cylinders.
R8. Torches shall be lit with friction lighters, stationary pilot flames or other safe sources
but not with matches.
R9. A permit-to-work certificate shall be obtained if the workplaces are of high fire risk.
R10. Proper shields shall be provided during welding and cutting works.
R11. Welders shall use suitable PPE and wear clothing that is free from grease, oil and
other flammable materials. Refer to the requirement of PPE in this Book of Safety
Standards. #

Safety : S1. Welding sets shall be kept clean and free from grease, oil, dust and dirt.
Precautions S2. Hose shall not be kinked or entangled, stepped on, run over or otherwise damaged.
S3. Cylinders shall not be recharged on site or warmed up.
S4. A ventilation system shall be provided if the natural air movement is not sufficient to
remove the exhaust effectively.
S5. Fire extinguisher shall be in place.
