This letter is written to kindly make you aware that there is inconsistency between the student’s
school records and the Birth Certificate from the civil registrar submitted to his adviser. MOKHTAR
BA RAMIL was written instead of MOKHTAR ALIE TITTING shown in his birth certificate.

I believe the reason or cause of this problem might be the birth certificate issued by the Malaysian
Government upon enrolling in the school. The last name of the child was taken from his father’s first
name, as customary to Malaysians. I sincerely request you to look into this matters it will only help
serving our students better.

Attached herewith is the photocopy of the birth certificates issued by Philippine and Malaysian
governments, elementary school records, and secondary school records.
