We understand that the California Engineering Contractors and Silverado Contractors have formed a joint venture (CEC/S JV) and have been
issued a project permit by Cal/OSHA for demolition of the East Span of the Bay Bridge from Yerba Buena Island to Pier E4 in the City and County
of San Francisco, per Caltrans Contract 040120T4.

The project will involve the exposure of workers to lead coatings on the existing bridge steel structure from a variety of demolition activities. Lead
containing debris will also be generated. We understand that the project is already underway and will be completed in approximately three years.
We further understand that work will be done under the direction of a Lead Compliance Plan, in accordance with Title 8 California Code of
Regulations, Section 1532.1 and that a Lead-Work Pre-Job Notification has been filed with Cal/OSHA. Blood lead testing will be completed for
workers as part of the Lead Compliance Plan medical surveillance program.

The Occupational Lead Poisoning Prevention Program (OLPPP) in the California Department of Public Health is responsible for investigating

Notes: elevated blood lead levels (BLLs) in workers. Our role is to assist employers in identifying and correcting work practices that can result in
employees being overexposed to lead.

Blood lead testing results are reported by laboratories to OLPPP's statewide adult blood lead registry. Several results for workers assigned to the
CEC/S JV have already been received by the registry. We presume that these may be pre-job testing results for your employees and that
additional BLL testing of workers will occur at regular intervals during the duration of the project based on air monitoring results and individual
worker results, in accordance with the lead standard. Frequently, laboratories do not accurately collect employer information at the time of blood
lead testing, and therefore workers who receive a BLL test may not be associated with their employer in our records. In order to accurately attribute
the CEC/S JV BLL testing performed to date and for the duration of the project, please provide the following:

1. Job roster for CEC/S JV employees, indicating the employee's name, date of birth, home address, and job title. As the roster changes during
the course of the project, we would appreciate periodic updates (e.g., monthly or quarterly).
2. Contact information for any subcontractors to the CEC/S JV that will complete project work tasks with potential exposure to lead.

In addition, please provide a copy of the Air Monitoring Plan and Demolition Plan for the project.

OLPPP staff assisted both firms comprising the CEC/S JV during the demolition of the Carquinez Bridge demolition project in 2006, and provided
feedback on air sampling, personal protective equipment, lead medical surveillance, worker hygiene and training. We can provide consultation to
you, your subcontractors(s), and/or consultant(s) by phone or in person on lead-related issues during the course of the demolition project for the
East Span of the Bay Bridge, if desired.

Note that during the Carquinez Bridge project, Industrial Hygienist, Dan Hernandez, did a representative pilot industrial hygiene study for the
purpose of verifying appropriate respiratory protection equipment selected for various job tasks/positions, as designated in the initial determination.
The CEC/S JV should do the same for this project, instead of relying on previous air monitoring data for the Viaduct Demolition project for initial
respiratory protection selection. The lead standard requires that previous air monitoring data used as the basis for an initial determination may only
be used where the employer has lead exposure data obtained within the past 12 months during closely similar workplace operations, and bulk
sampling results for both projects are similar. Bulk sampling results should be representative of actual coated steel surfaces that will be disturbed
during project demolition activities; Caltrans data may not represent specific areas of planned disturbance.

Yes, we were quite concerned about the two workers (and two others that were slightly over single digits) and did an extensive investigation. We
think we identified, and have implemented, several items we could improve on and are doing follow up blood draws this week. I'm headed down to

Regarding the s, their exposures leading up to the separation of the bridge in April this year were more moderate. The actual burning was
intermittent and shorter duration. After April the work was intense and constant. When we saw the big jumps, we immediately analyzed their work
practices, PPE and exposures. We identified several areas we could improve. For the most part, we were able to arrest the increases and are
seeing moderate declines. We continue to explore enhancements to improve engineering controls, scheduling, PPE and hygiene practices. We
are confident we can get even the highest exposed workers under 10 µg/dl (BLL) with aggressive oversight and support.

Talked to Mr. McKinney on 8/5 and he assured me that all employees on the roster he provided had been blood lead tested and the info was
accurate. Compared the previous roster to this one and looked again in ELVIS and found the 3 workers; these are all now correctly associated with
the employer. Following the phone call, Mr. McKinney requested that I send him the Dec 2013 roster (completed).

This employee was recently hired in September onto our temporary steel crew. This crew performs grinding, welding, rivet busting, and cutting to
prepare areas of the bridge for new strengthening steel. Once the area is prepared, they also install the new steel. Specifically, this employee was
mostly performing grinding and rivet-busting (removal).

Once we received the high result, we immediately removed the employee from the work where there was exposure to lead and put him to work in
our yard. We also interviewed him to determine how his level reached so high a level when everyone else's levels were low. The employee
admitted to us that he was chewing tobacco during work. We feel that this practice is what caused the high lead level.

Additionally, our Field Safety Representative provided a refresher training session for that crew going over the exposure risk, work practices, use
and care of PPE, and hygiene.

Since the initial high lead level, the employee has had another blood sample taken, and his blood lead level went down to 3 . We will be sending
him to be seen next week by Dr. Stephen R. Nord, M.D. at Premier COMP Medical Group in Pleasanton, CA.

Hi Michael,

The last two BLL results for the worker (initials ) we discussed in December were 3 ug/dl. His BLL was going down around the end of 2015,
beginning of 2016. Has this employee gone back to the tasks he was doing in the fall, reverted to chewing tobacco while working, or has
something else changed?
The results that I've seen for other worker so far this year are all less than 25.

Michael,

You are correct that this employee was assigned to other work as previously stated. His blood lead levels went down to about 2 , so we decided
that he could return to the crew with confidence that his work/hygiene practices had improved and his levels would continue to go down. His blood
lead level went back up to 3 as you have been informed. We took a final blood draw last Thursday, his final day of working with lead painted
coated steel, and his results came in today at 3 . We have assigned him to other than lead paint disturbing work again, and he will no longer be
working with lead painted steel.

Michael,

1.We are still dismantling the bridge structure coated with lead paint. From time to time we will be torch cutting out on the bridge to remove various
members before the whole truss section can be removed. There is about 1 week left of this work.

2. Once the truss is removed, it is transported to our yard area in the Port of Oakland where it will be completely cut into pieces to be hauled to a
recycler. It will take about 1 week to completely cut up a truss section using torches, and we have 14 trusses left. A truss will be lowered about
every two to three weeks. We are lowering the first truss tomorrow, and the last truss should be lowered in late March or early April.

3. The last bit of lead work to mention is the removal of paint from selected steel members using water blasting method. These members were
selected by Caltrans and will be used in various art projects. This work has been going on for several months and will continue for several months
as well.

1.We are still dismantling the bridge structure coated with lead paint.  From time to time we will be
torch cutting out on the bridge to remove various members before the whole truss section can be
removed.  There is about 1 week left of this work.
 
2. Once the truss is removed, it is transported to our yard area in the Port of Oakland where it will be
completely cut into pieces to be hauled to a recycler.  It will take about 1 week to completely cut up
a truss section using torches, and we have 14 trusses left.  A truss will be lowered about every two to
three weeks.  We are lowering the first truss tomorrow, and the last truss should be lowered in late
March or early April.
 
3. The last bit of lead work to mention is the removal of paint from selected steel members using
water blasting method. These members were selected by Caltrans and will be used in various art
projects. This work has been going on for several months and will continue for several months as
well.

You are correct that this employee was assigned to other work as previously stated.  His blood lead
levels went down to about 2 , so we decided that he could return to the crew with confidence that
his work/hygiene practices had improved and his levels would continue to go down.  His blood lead
level went back up to 3 as you have been informed.  We took a final blood draw last Thursday, his
final day of working with lead painted coated steel, and his results came in today at 3 .  We have
assigned him to other than lead paint disturbing work again, and he will no longer be working with
lead painted steel.

The last two BLL results for the worker (initials ) we discussed in December were 3 ug/dl.  His BLL
was going down around the end of 2015, beginning of 2016.  Has this employee gone back to the
tasks he was doing in the fall, reverted to chewing tobacco while working, or has something else
changed?
The results that I’ve seen for other worker so far this year are all less than 25.

Michael,
 
This employee was recently hired in September onto our temporary steel crew.  This crew performs
grinding, welding, rivet busting, and cutting to prepare areas of the bridge for new strengthening
steel.  Once the area is prepared, they also install the new steel.  Specifically, this employee was
mostly performing grinding and rivet-busting (removal).
 
Once we received the high result, we immediately removed the employee from the work where
there was exposure to lead and put him to work in our yard.  We also interviewed him to determine
how his level reached so high a level when everyone else’s levels were low.  The employee admitted
to us that he was chewing tobacco during work.  We feel that this practice is what caused the high
lead level.
 
Additionally, our Field Safety Representative provided a refresher training session for that crew
going over the exposure risk, work practices, use and care of PPE, and hygiene.
 
Since the initial high lead level, the employee has had another blood sample taken, and his blood
lead level went down to 3 .  We will be sending him to be seen next week by Dr. Stephen R. Nord,
M.D. at Premier COMP Medical Group in Pleasanton, CA.

You can see that many of the employees are no longer active.  Steel demolition portion of the
project is over, and many employees have been laid-off as a result.  The only remaining exposures
left on the project are for our subcontractor that is blast-cleaning some of the steel members for
Caltrans to use in various art projects.  California Engineering/Silverado Contractors employees are
not working with or around any lead any more.

Thank you. It was nice to finally meet you and the crew, and I’m glad you braved the rain and
