AFFIDAVIT OF ACKNOWLEDGMENT

I, GODOFREDO N. OLIVA, of legal age, Filipino, widower, and resident

of Brgy. San Jose, Caloogan, Palo, Leyte Philippines, after having been duly sworn
to in accordance with law, hereby depose and say:

1. That I am the absolute and legitimate owner of a parcel of land located in Brgy.
Caloogan, Palo, Leyte as evidenced by Tax Declaration No. 08-30-0013-00145
and more particularly described as follows:

“A PORTION of a parcel of land , with the building and

improvements thereon, situated in the Barrio of Caloogan, Palo,
Leyte, Philippines. Bounded on the North by Lot # 7329; on the
South by a Barangay Road; on the West by Lot # 7331 and the
East by Bahay Creek with an area of ONE HUNDRED SQUARE
METERS(100 SQ. M.) more or less.”

2. That on_____________________, I executed a Deed of Absolute Sale for the

above property in favor of Jireh Mark E. Cruiz for the amount of EIGHTY
THOUSAND PESOS(Php80,000.00);

3. That I am executing this Affidavit to attest to the truth of the foregoing facts and
for whatever legal purpose this may serve.
