1. Updated Personal Data Sheet (2 sets)

Date of submission: January 29, 2014
Please observe the following in accomplishing your PDS:
a. Provide all information being called for. If the information is not applicable to you,
write not applicable or N/A.
b. The ID picture must have the following specifications:
1. Passport size (4.5 x 3.5 cm or 1.8 x 1.4 inches)
2. Colored with white background
3. Taken within three (3) months prior to the date of accomplishment
4. Printed on good quality photo paper
5. In bare face (with no eye glasses or any accessories that may cover the facial
features
6. Taken in full-view directly facing the camera
7. With full and handwritten (not computer-generated) name tag in the format Last
Name, Middle Initial, First Name and Name Extension (if any)
c. For the trainings/seminars attended, start from the most recent training and use
separate sheet if necessary.

2. Authenticated True Copy of Board Rating (3 copies)

Date of submission: February 28, 2014
The copy must be authenticated by the issuing agency (e.g. PRC) and must bear
original signature. It should not be a copy of an authenticated one.

3. Authenticated True Copy of Updated License (3 copies)

Date of Submission: February 28, 2014

The Civil Service Commission has announced a visit to all State Universities and
Colleges in the region sometime in March 2014 and the abovementioned documents
will be checked.
