Floor)
1) Submit the following to the Office of Admissions and Scholarship
College Applicants
a) Application Form @ OAS
b) OLSAT and RTCU/ATCU Results (get them from the College Guidance Center)
Bp. Pueblos Senior High Applicants (Grade 11 & 12)
a) Application Form @ OAS
b) Endorsement from the Bp. Pueblos Senior High Principal

1) 1 copy of close-up 4R size color photo (high quality/resolution) light make up only
2) 1 copy of whole body 4R size color photo (high quality/resolution)
(NOTE: Studio taken photo is highly required.)
3) Brief Interview
