Greetings!

The Accreditation Process of the National Health Insurance Program ensures that the qualifications and
capabilities of health care providers are verified in accordance with the guidelines, standards and
procedures set by the Corporation.

In line with this, I, as a Medical Officer of Davao del Sur Provincial Hospital had been in compliance with
such requirement to assure that health care services rendered are of the desired and expected quality.
This approved re-accreditation was valid from February 13, 2015. However, on September 27, 2017 this
accreditation expired. Personally, I made an effort to comply with the requirements for re-accreditation
but this effort proved futile as I was not able to complete the process for renewal. And now, I am faced
with the circumstance of PhilHealth claims signed under my name not to be reimbursed due to the
expiration of my accreditation.

Words cannot express how sorry I am for this shortcoming on my part. My failing caused many problems
for the hospital and to your institution as well. It was such an offhand misstep that never should have
happened. I apologize for all of the problems, and I hope to be able to atone for my mistake.

One of the most important aspects of my job is to be vigilant and ensure that not only are healthcare
services given with quality but also that such accreditations required are tracked. My attention faltered,
and that turned into a much bigger problem. Now, I appeal to your good office to continue granting the
reimbursements for the claims under my name as I am now in the process of complying for the required
re-accreditation.I have tried my best to solve the problem.

I also will make sure in the future never to repeat this error. I will be extra cautious when mailing the
tickets. I will also not let my attention wander again, so that I will not have another careless mistake on
my record. I really love my job here at the Playhouse, and I want to continue working here. Although I
cannot take back my actions, I hope that this letter and my retributions will help to fix the problems I have
caused.
