1. Why is the boy crying everytime when he urinate?

Normally, until the boy was 3 or 4 years old, the penis will grow, then the debris which
produce by preputium epitel was collect in the preputium, it will separate between prepuce
and glan’s penis. Maybe in the scenario  there was adhesion between prepuce and glan’s
penis so the prepuce became narrow  the urin will flow abnormally.

2. Why is the penis looked reddish?

Maybe the prepuce get pulled back to posterior,  glans penis was trapped  the flow back
(reflux) of vena superficial, but the artery stay of normally  the penis get edem so the
penis get the coloring like a red and the patient get hurt.

Reddish : inflamation sign . normally, when the baby was born ther was adhesion between
prepuce and glans penis. If the prepuce wasn’t separated, the debirs was collected  make
the iritation  inflamation

3. Why is the penis balloning everytime he urinate?

The foreskin become trapped behind the glans penis and can’t be pulled back to it’s normal.
 there was a small space between the prepuce and glans penis  urine can’t flow to OUE
rapidly, but trapped there. And the vein getting trapped.

After infection (reflux)  OUE make the fibrin (scars)  OUE getting smaller

4. Why the balloning penis is getting bigger?

There was adhesion  the urin was collected in the space between prepuce and glan’s
penis.  acumulation from the preview urin.

5. Why the boy didn’t have fever and there was no history of trauma?
There was no infection so the body was not increase the thermostat. It was congenital
anomaly. Infection : scars  obliterasi
 Trauma : glans penis become behind
6. What is type of congenital anomalies in urinary tract?
Hypospadia : the OUE is located in ventral and proximal of the penis
Tortio testis : a condition when funiculus spermaticus was rotate
Phimosis : a condition in males where the foreskin (prepuce) cannot be fully retracted over
the glans penis.  OUE is getting smaller
Paraphimosis : the prepuce retracte until sulcus coronarius
Epyspadia : OUE is located on dorsal penis
Balanitis : the inflamation on the glans penis. Maybe the smegma has no cleaning the debris,
or the old man has no circumcition

manual retracting  Steroid cream 2 weeks before sirkumsisi  circumsision

1. prevent the infection on glans penis and prepuce
2. Prevent the accident of carcinoma

8. DD?
Phimosis is when the OUE was small, and there was persisten adhesion between prepuce
and glans penis, and prepuce can’t be retracted to posterior.
Phimosis can caused by low hygiene when miction, so there was collecting of debris, many
bacteries can make infection in glans penis.
Infection  fever and decrease the appetite and growing period
Etiology : congenital anomaly, hygiene, infection, after the unhygiene circumsision
Therapy
Analgetic to reduce inflamation  salep or puyer
manual retracting  Steroid cream 2 weeks before sirkumsisi  circumsision
