Instructions

1. This survey applies to a water service provider that provides the water requirements of a community of at least 15 households.
2. Fill up this form in 2 copies. Keep Copy 1 of the completed form for your files. Submit Copy 2 to your Municipal/City Planning and
Development Coordinator if you are not able to encode the contents of this form in the ListahangTubig website.
3. Use data for the calendar year ended December 31, 2017.

Water Service Provider Data Section


Box 1. Management Type Refer to these definitions.
 BWSA Non-stock, non-profit organizations envisioned to operate and manage Level I water
supply facilities.
 RWSA Community-based water user’s association formed to manage piped water supply
systems either with house connections (level 3) or a network of public taps (Level 2).
 Cooperative A membership organization formed under the Cooperative Code of the Philippines to
operate and maintain water supply systems and registered with the Cooperative
Development Authority.
 Homeowners’ An organization that operates and maintains a water supply system and is registered
Association with the Securities and Exchange Commission or Housing and Land Use Regulatory
Board.
 LGU-Run Utility A water supply system owned and operated by the provincial, city or municipal
government.
 Water District A quasi-public corporation formed by the local government unit under the Provincial
Water Utilities Act for the operation and maintenance of water supply and wastewater
management system, which has been issued a Certificate of Conditional Conformance
by the Local Water Utilities Administration.
 Real Estate A real estate developer operating the water supply system that provides potable water

 Unnamed water An unnamed water service provider is one that serves at least 15 households and
service provider which is not registered formally with any government agency.
 Industrial Locator An industrial estate operating the water supply system in an economic special zone to
provide water to its locators.
 Peddler Non-pipe water service provider operator extracting water and supplying/delivering
water by container.
 Ship Chandler A water supply operator providing water to ships.
 Other Private Sole proprietorships, corporations and other private entities formed under the general
Operator business and corporation laws of the country for the operation and maintenance of
water supply systems.
Box 2. Water Service Use the official name of the water service provider.

Box 3. Year Started This refers to the year the water service provider started operations.

Box 5. Level of Service Refer to these definitions.


 Level 1 only Point source
A protected well or a developed spring with an outlet but without a distribution system
as it is generally adaptable for rural areas where the houses are thinly scattered serving
an average of 15 households with people having to fetch water from up to 250 meters
distance
 Level 2 only Communal faucet system or stand post
A system composed of a source, a reservoir, a piped distribution network, and
communal faucets. Usually, one faucet serves 4 to 6 households. Generally suitable for
rural and urban fringe areas where houses are clustered densely to justify a simple
piped system
 Level 3 only Waterworks system or individual house connections
A system with a source, a reservoir, a piped distribution network and household taps. It
is generally suited for densely populated urban areas.
 Levels 1 and 2 Levels 1 and 2 systems co-existing in the service area with predominance of Level 2.
 Levels 2 and 3 Levels 2 and 3 systems co-existing in the service area with predominance of Level 3.

recommended that he create an email account. If no email address can be provided, indicate “none” in this box.

Box 14. Mobile Number Provide the contact person’s mobile number.
Box 15. Fax Number Provide the contact person’s fax number.

Service Area and Population Served Section


The water service provider may be serving more than one barangay. In such a case, fill up one line for each barangay covered and the
corresponding households that the water service provider is serving. If the water service provider is serving just a portion of the barangay, just fill
up the name of the barangay and the number of households served.

Add a separate sheet if there are more barangays served.


Box 16. Region These refer to the region or province city or municipality where the service area of the water service provider is

Volume of Water Extracted Section


The water service provider may have more than one water source. Fill up all the requested information for each water source across the row.

Box 21. Water Source – Bulk water source refers to raw water or treated water purchased by the water service provider for distribution to

producing water for sale to another water service provider.

Water Source – Rain water harvesting refers to accumulating and storing rainwater for reuse for basic water needs.
