2. Fill up this form in 2 copies. Keep Copy 1 of the completed form for your files. Submit Copy 2 to your Municipal/City Planning and
Development Coordinator if you are not able to encode the contents of this form in the ListahangTubig website.
3. Use data for the calendar year ended December 31, 2017.

Box 21. Water Source – Bulk water source refers to raw water or treated water purchased by the water service provider for distribution to
