RE: CANCELLATION OF CREDIT INSURANCE AND REFUND

GENTLEMEN:

This is referring to the credit card purchased by __________________in

_______________________the account number __________________________.
On August 7, 2018, our client found out that she has been deducted PHP 500.00 per month for
almost one year in her credit card. According to our client she was not aware of that credit
insurance and she was not entered into any policy insurance so therefore it should be VOID from
the beginning. The Metrobank femme visa card is not authorized to deduct any amount without
the consent of the borrower.
In view thereof, our Client is asking for your consideration to cancel the credit insurance and to
refund the amount she paid.
Hence, FINAL DEMAND is hereby made upon you to take an action with this matter. Otherwise
to protect the Interest of our client, we shall be constrained to file appropriate legal action
against you.
We trust that you will take this matter with utmost importance to avoid inconvenience.
