I have been informed of my role in the project entitled……………………………………..

and agree to the following:

 My work may be distributed on YouTube and may be used in the Visual Arts
Exhibition.
 I will be available to work on the dates scheduled and will inform the production
crew if I cannot attend at least 24 hours in advance
 I will not make any drastic changed to my appearance during the shooting period
e.g. hair colour and cuts
 I will be punctual, reliable and co-operate with what needs to be done.

In return the project manager will:

 Inform of any time or location changes 24 hours in advance

 Will provide a call sheet with information regarding the shoot 24 hours in advance
 Will provide all props, make up and costume required.
