AFFIDAVIT

I, ROLLY OLIQUINO y ESPINOL, of legal age, presently

residing at 243 Sanchez St., Brgy. Tinajeros, Malabon City , after being
sworn in accordance with law, hereby deposes and states that:

1. I was charged for Violation of City Ordinance 03-

2003 (Drinking in Public Places) filed before the
Metropolitan Trial Court Branch 56, under Criminal Case
Number 297-18. However, due to inadvertence, and
excusable negligence, my name, as written in the
Information was written as ROLLY ESPINOL y
OLIQUINO;

2. I was the one who personally appeared before the

proceedings of the aforesaid case until the same was
terminated;

3. In view of the foregoing, I am executing this affidavit,

to attest that ROLLY ESPINOL y OLIQUINO as
appearing in the records of the Metropolitan Trial Court
Branch 56, under Criminal Case Number 297-18 and
ROLLY OLIQUINO y ESPINOL are one in the same
person to avoid further confusion for whatever purpose
this affidavit may duly serve.

IN WITNESS WHEREOF, I have hereunto affixed my
