JOINT AFFIDAVIT OF DISCREPANCY

WE, VIOLETA M. DEQUILLA, of legal age, Filipino and a resident of Purok

Maghili-ugyon, Brgy. 35, Bacolod City, Negros Occidental, Philippines; and LEONIDA
M. BARTOLATA, of legal age, Filipino and a resident of Purok Mahimulaton, Brgy.
Banago, Bacolod City, Negros Occidental, after having been duly sworn in accordance
with law, hereby depose and say:

1. That we personally know ADELINA LANZA PARADERO, ERNEIL FILOMENA

LANZA, ERLINDA J. TOLECO and JOSE JERONILLA TOLECO, who were our
long-time neighbors and acquaintances at Brgy. 35, Bacolod City, Negros
Occidental;

2. That ADELINA LANZA PARADERO, ERNEIL FILOMENA LANZA and

FLORENDA TOLECO ARDIENTE are the beneficiaries of JOSE JERONILLA
TOLECO, who died intestate and without issue on June 15, 2018;

3. That several discrepancies appear on the records of ADELINA LANZA

PARADERO and ERNEIL FILOMENA LANZA;

4. That as per the E4 records of JOSE JERONILLA TOLECO, ADELINA LANZA

PARADERO was indicated as the “nephew” of the member instead of “niece”,
the latter being the true and correct relationship of ADELINA to JOSE;

5. That the name of ERNEIL T. LANZA as appearing in the E4 of JOSE JERONILLA

TOLECO and ERNEIL FILOMENA LANZA as appearing in the latter’s birth
certificate, refers to one and the same person, the latter being his true and correct
name;

6. That ERNEIL FILOMENA LANZA used the middle name TOLECO in his SSS
records as the same is the maiden last name of her mother known to him, but
unfortunately his birth certificate did not reflect his true middle name as the last
name FILOMENA was the last name of her grandmother;

7. That for purposes of convenience and synchronizing his government records, he

nevertheless used the last name of her grandmother as appearing in his birth
certificate which is FILOMENA;

8. That we further declare that the names LINDA GERONILLA FILOMENA as

appearing in the birth certificate of ERNEIL FILOMENA LANZA, and ERLINDA G.
TOLECO as appearing in the marriage contract of ADELINA LANZA PARADERO,
refers to one and the same person;

9. That we further declare that LINDA GERONILLA FILOMENA, ERLINDA G.

TOLECO and ERLINDA J. TOLECO, refers to one and the same person, the latter
being her true and correct name;
 10. That the last name FILOMENA is the maiden last name of their grandmother,
which was probably erroneously entered when the certificate of live birth of
ERNEIL was filled out;

11. That the above-mentioned discrepancies must have been brought about by
excusable neglect or mere inadvertence;

12. That we are not related to them either by consanguinity or affinity;

13. That we are executing this affidavit to attest to the truth of the foregoing
statement and for whatever legal purpose it may serve.
