To Whom It May Concern:

Within twelve (12) months of the date described above, this Notice is being sent pursuant to the
requirements of O.C.G.A. § 36-11-1, to provide you with this notice of a tort claim . You are
hereby notified you have a thirty (30) day opportunity for adjustment of a tort claim against Cobb
County as a result of the following incident:

(a) Name of the Government entity involved: Cobb County Adult Detention Center;
(b) Time: Beginning on 10/07/2-014 through the present ;
(c) Place: Cobb County, Marietta, Georgia;
(d) Nature of Loss Suffered: sexual battery and mental anguish and trauma;
(e) Amount of Loss Claimed: in excess of $25,000.00 and for permanent impairment;
(f) Acts or omissions which caused the loss: Against Sheriff Neil Warren: Sexual Battery,
Intentional and Negligent Infliction of Emotional Distress against Cobb County Adult Detention
Center (Respondeat Superior), Agency Liability, unlawful detention, failure to provide access to
trained medical personnel before and during treatment, conspiracy, retaliation, Negligent
Infliction of Emotional Distress, Negligent Hiring, supervision and retention.

No action to make a civil recovery for these claims will be commenced except upon the
expiration of thirty (30) days following receipt of this notice, or the County’s denial of the claim,
which ever occurs first.
