Los estudios transversales habitualmente no se hacen en un solo
día, incluso pueden llevar meses o años para su realización y durante
la investigación se estudian varios sujetos, pero todos ellos en un
momento determinado de su vida. No se los sigue en el tiempo. Es
importante señalar que el corte transversal representa un momento
en la vida de la persona que está siendo estudiada o entrevistada,
no hay un seguimiento de esa persona. Debido a que los estudios
transversales enfocan un punto en el tiempo, se llaman también de
prevalencia,5,6 y permiten calcular la prevalencia de una enfermedad, no
así su incidencia.
La información puede usarse tanto en forma descriptiva, para
mostrar cuánto hay de cada característica en la población o para apoyar
la generación de hipótesis al analizar asociaciones existentes entre
algunas características de los sujetos. Si la variable estudiada es de tipo
cualitativo (por ejemplo: una enfermedad o sus síntomas), los resultados
del estudio de prevalencia generalmente se entregan como porcentajes
(tasas de prevalencia). Las variables cuantitativas pueden expresarse a
través de medidas de resumen, como promedio y desviación estándar,
dependiendo del tipo de variable o pueden categorizarse con base en
los criterios clasificatorios previamente establecidos.
Un estudio de prevalencia puede ser también analítico cuando se
examinan las variables de interés en subgrupos de sujetos, entonces
pueden estudiarse hipótesis causales o de asociación. Sin embargo,
este tipo de modelo de investigación solo permite probar hipótesis
simples, a saber, si el promedio de una característica difiere de un
valor estándar conocido o del de otra población. También permite
la comparación entre grupos o de un mismo grupo en diferentes
momentos, por ejemplo en años o periodos diferentes.6,7
Los estudios transversales tienen escaso valor para el análisis
de enfermedades agudas, ya que ellas se caracterizan por su corta
evolución y su naturaleza rápidamente cambiante; en cambio, son

del total de la muestra evaluada, 187 personas (4.7%, IC 95% 4.4 a 5.8)
refirieron la presencia de este problema. La edad media de los sujetos
encuestados fue de 49.5 ± 13 años, predominaron las mujeres (67%),
la mayoría dedicadas al hogar (26.2%) .
Se dividió a la República Mexicana en ocho zonas geográficas; se
encontraron variaciones de acuerdo con la localización y se observó
una mayor prevalencia en la zona Oeste (Nayarit, Jalisco, Colima y
Michoacán), con 20.1%. En relación con la escolaridad, la IF se presentó
con mayor frecuencia en personas con alto nivel de escolaridad
(49.9%); los menos afectados fueron aquellos con escolaridad baja:
primaria, 11.6% y secundaria, 12.1% (Cuadro 4). Sin embargo, al hacer
un comparativo con controles pareados por edad y por sexo, en el
análisis multivariado, se encontró que una edad mayor, ser ama de casa
y tener bajo nivel de escolaridad son factores asociados a incremento
de riesgo de presentar episodios de IF.
La calidad de vida se evaluó utilizando un cuestionario específico para
síntomas gastrointestinales (PAGI-QoL) que evalúa cinco dominios:
a) actividades diarias, b) vestido, c) dieta, d) relaciones personales y
e) aspectos psicológicos. En comparación con la población control
(síntomas gastrointestinales negativos), los pacientes con IF mostraron
puntuaciones más bajas en todos los dominios, excepto en el de
relaciones personales (Cuadro 5).
4.1. Proctalgia crónica y fugax. Del total de pacientes encuestados,
245 cumplieron con criterios diagnósticos para proctalgia crónica, lo
que traduce un 6.2% (IC 95% 5.5 a 7.1,) de la población estudiada. Por
otra parte, 311 pacientes reportaron proctalgia fugax, es decir, 7.9%
(IC 95% 7.1 a 8.8) del total de encuestados. La media de edad de los
pacientes con proctalgia crónica fue de 42 ± 12, significativamente más
alta al compararla con la de los sujetos con criterios para proctalgia
fugax (38 ± 18, p < .05).
En el análisis univariado de las variables investigadas en las diferentes
zonas geográficas, se observó que a pesar de que en la zona Oeste y
Centro Sur se presentaron la mayoría de casos de proctalgia crónica
y fugax no hubo diferencia en comparación con la población control.
Así mismo, no se observaron diferencias en relación con el estatus
socioeconómico. Sin embargo, sí se encontró diferencia significativa
(p < .05) en prevalencia de encuestados que realizan labores del

al inicio del estudio 54% de los pacientes con ERGE tenía dispepsia
y 62% de los dispépticos tenía ERGE. Tras 5 años, solo 25% de los
pacientes inicialmente diagnosticados con dispepsia desarrolló ERGE.10
La prevalencia de sobreposición ERGE-DF en el estudio de Locke
fue de 7.5% (IC 95% 5.2 a 9.8).4 Dos estudios provenientes de Asia
reportaron prevalencias menores que los trabajos occidentales.11,12 Un
trabajo coreano que estudió 1,688 sujetos encontró prevalencia de
ERGE de 4.6%, de dispepsia 6.3% y de sobreposición ERGE-dispepsia
de 2.3% (IC 95% 1.4 a 3.0), con una mayor asociación de sobreposición
en personas con ansiedad.11 Finalmente, un estudio japonés con 2,680
sujetos observó una prevalencia de 7.7% de ERGE y 10% de DF, con
sobreposición ERGE-DF de 1.3%.12
2.2. ERGE y SII. En los últimos 20 a 25 años la evidencia ha apoyado
cada vez con mayor frecuencia la sobreposición entre ERGE y SII.
Algunos estudios han descrito que entre 30% a 71% de los pacientes
con SII tienen pirosis y que la prevalencia de SII en ERGE es mayor que
en la población general.13,14 Otros trabajos han calculado la prevalencia
de SII en 5.1% al eliminar de la población sujetos con ERGE. Una revisión
sistemática publicada en 2006, que incluyó 15 publicaciones, determinó
una prevalencia máxima promedio de ERGE de 39.3% en pacientes
previamente diagnosticados con SII. En siete estudios que evaluaron
la prevalencia de SII en sujetos previamente diagnosticados con ERGE,
ésta fue de 48.8%. Con base en la prevalencia de SII (12.1%) y de ERGE
(19.4%) los autores determinaron la prevalencia de sobreposición entre
ambas en 5.1%, excediendo la prevalencia individual de cada entidad
por separado.15 En el trabajo de Locke la prevalencia de sobreposición
ERGE-SII fue similar: 5.6% (IC 95% 3.5 a 7.7),4 pero en el de Lee fue
menor: 2% (IC 95% 1.2 a 2.6),11 similar a la de otro grupo asiático
proveniente de Corea.16
Otros estudios han confirmado una asociación recíproca entre
ambas entidades. Kennedy et al reportaron que 19% de los pacientes
con ERGE tenía síntomas de SII, y 32% de aquellos con SII, tenía ERGE.17
De Vries también observó que el SII es más prevalente en personas
con ERGE que en la población general (35% vs. .6% a 6%),18 al igual que
Pimentel (71% vs. 35%).13 Zimmermann evaluó la prevalencia de SII en
ERNE, y encontró que la mitad de los pacientes cumplía criterios de SII
(Roma I), aunque sin encontrar una relación directa entre exposición
