AFFIDAVIT OF DISCREPANCY
(Re: Middle Name)

I, CHRISTINA DAWN DEGAWEN DELA CRUZ, of legal age, married, Filipino

citizen and with residence and postal address at Patay, Poblacion, Sagada, Mountain
Province, after having been duly sworn to in accordance with law, do hereby depose
and say that:

1. That I am a holder of a Voter’s Identification Card bearing number

4409-0002B-L2581CDD20000 issued by the Commission On Elections
(COMELEC). Attached hereof is a photocopy of my Voter’s
Identification Card and hereto Marked as ANNEX “A” for your
ready reference ;

2. That due to inadvertence, my Middle name in the aforesaid Voter’s

Identification Card was erroneously entered as “ DEGAWAN”, instead of
my true and correct Middle Name which should have been “DEGAWEN”
as evidenced by my Certificate of Live Birth. Attached hereof is a copy
of my Certificate of Live Birth and hereto Marked as ANNEX “B”
for your ready reference ;

3. That I am planning to apply for the renewal of my passport and the only
Valid Identification Card that I have with me is my Voter’s Identification
Card and as such, I am attesting that the middle name “DEGAWAN” as
appearing in my Voter’s Identification Card is erroneous for my true and
correct Middle Name is “DEGAWEN” as appearing in my said Certificate
of Live Birth and Old Passport. Attached hereof is a copy of my Old
Passport and hereto Marked as ANNEX “C” for your ready
reference ;

4. That the said error is merely clerical which can be corrected in

conformity with the entries in my Certificate of Live Birth without any
court proceedings;

5. That I am executing this affidavit for the purpose of establishing the

foregoing facts and for all legal intents and purposes it may serve;
