SPOT REPORT ON FATAL-VEHICULAR TRAFFIC INCIDENT (LAOAG CPS).

At about 09:10 PM of December 24, 2017, elements of Laoag CPS responded to a report of a
vehicular traffic incident happened along Vintar Road, vicinity of Brgy. 54-A, Lagui, Laoag City this
province with GC: Lat 18.208186 Long: 120.629663 involving a Isuzu Elf Garbage Truck
Compactor bearing plate no. RKY420 registered under the City Government of Laoag driven by
Jerome Manuel y Amian, 39 years old, City Employee of Laoag, holder of PDL and a resident of
Brgy. 61-A, Caaoacan, Laoag City, this province and a Honda TMX makeshift tricycle, no plate
attached driven by Nelwin Baay, 35 years old and a resident of Brgy. Esperenza, Vintar, Ilocos
Norte. Investigation conducted disclosed that prior to the above stated time and date, said garbage
collector truck was parked by the driver unattended on the right lane facing north direction along the
said road due to mechanical trouble with branches of tree as improvised early warning device while
the driver and his volunteer garbage collector identified as Joselito Borja y Lorenzo, 48 years old,
single and a resident of Brgy. 1, Laoag City, this province were sitting on the rear right side of said
truck waiting for a mechanic to trouble shoot the truck. At that instance, a speeding multi-purpose
tricycle heading north bumped the said garbage truck. As a result, the driver of said improvised
tricycle together with his passenger identified as Marlon Andres y Bitanga, 25 years old, single,
and a resident of Brgy. Isic-isic, Vintar, this province and Joselito Borja sustained severe
injuries on their bodies and all rushed to GRASMH, Laoag City by the Laoag City Paramedics for
medical treatment but Marlon Andres pronounced dead on arrival by the attending physician Dr.
Recarte Caday due to head injury. Further, based on the result of alcoholic breath test, Marlon
Andres found to have positive in alcohol with vol. of 0.494 mgl. The garbage truck incurred minor
damage while the multi-purpose tricycle incurred heavy damages and now under the custody of
Laoag CPS for proper disposition. IOC-Brodlee Villanueva with CP No. 09177062545. Progress

2ND PROGRESS REPORT ON HACKING INCIDENT (DINGRAS MPS).

This pertains to the hacking incident transpired at about 7:20 PM of December 21, 2017 at Brgy.
Espiritu, Dingras, Ilocos Norte (Lat 18.090548 Long 120.664276) wherein the victim was Quirino
Lorenzo y Balagat, 81 years old, single, farmer was hacked by the suspect identified as Rogelio
Agustin y Apostol, 72 years old, married, farmer, both residents of aforesaid place. Be informed
that a case of Frustrated Homicide against the above-named suspect was referred at the
Provincial Prosecutors Office, Laoag City as inquest proceedings docketed under NPS
No.I-04-INQ-17I-00287. During the inquest proceeding, Rommel G Calupig, Assistant Provincial
Prosecutor issued an Order of Release of the above-named suspect dated December 23, 2017 due
to the reason that the medical certificate of the victim has not been submitted because of the
unavailability of the doctor who assisted him. Further, said case will be filed thru regular filing upon

forwarded to that office as soon as obtained

SPOT REPORT ON DROWNING INCIDENT (VINTAR).

At about 7:00 AM of December 2017, elements of Vintar MPS proceeded at Brgy. Parparoroc,
Vintar, Ilocos Norte to verify the veracity of the report that an alleged victim of drowning incident
was discovered thereat particularly at the irrigation canal. Investigation conducted disclosed that at
about 6:30 AM of same date, Johnrick Rebondo y Pasion, 24 years old, single, and a resident of
aforementioned place discovered the lifeless body of Consolacion Espiritu y Mangoma, aka
“Sion”, 75 years old, single, a resident of said place at the irrigation canal. Further investigation
conducted disclosed that the victim is mentally incapacitated. The physician of Vintar Municipal
Health Officer conducted post mortem examination upon the cadaver of the deceased and
declared that the cause of her death is due to asphyxia secondary to drowning. The physician also
declared there is no indication of foul play. First Responders/Patrollers: SPO1 Onofre Loreto V

FINAL REPORT ON DIRECT ASSAULT, PHYSICAL INJURY AND OTHER LIGHT THREAT
(DINGRAS MPS).

This pertains to the direct assault, physical injury and other light threat that transpired at about
12:10 AM of December 23, 2017 at the Happiness Videoke and Carinderia owned & managed by
Emee Mangapis y Deleon situated at Brgy. Madamba, Dingras, this province wherein the suspect
was identified as Oswaldo Parado III y Castro aka “SHANE”, 64 years old, married, unemployed
and a resident of said place. Be informed that cases of Direct Assault, Physical Injury and Other
Light Threats were referred at the Provincial Prosecutor’s Office docketed under NPS No.
I-04-INQ-17L-00288. However, the private complainant Norma Ecnas manifested her total loss of
interest in the further prosecution of her complaint against the respondent. The case was dismissed
and subsequently, respondent was released from the custody of Dingras MPS by virtue of a Release
Order signed by Hon. Rommel G Calupig, Assistant Provincial Prosecutor and Officer In-Charge.

FLASH ALARM ON MISSING PERSON (BADOC MPS).

Request pass to all PDs, PRO1 except PD, INPPO to look for and take into custody a male person
named Gregorio Inez y Bermus, 47 years old, single, person with disability (mongoloid) and a
resident of Brgy. Caraitan, Badoc, Ilocos Norte who was reported missing by Jennifer Eccomienda
y Garonia, 33 years old, married, and a resident of same barangay. Reporter averred that her
brother in-law left their residence at about 9:00 AM of December 23, 2017 without informing them
where to go and never returned home up to this time and date. Further, the reporter stated that
they exerted all their diligent efforts to locate and know his whereabouts but turned futile, hence,
they considered him missing. If found, request inform Jennifer Encomienda with CP#
09977805147 and Badoc MPS thru Hotline No. 670-0150 & 09154488965 for proper disposition.

PROGRESS REPORT ON MISSING PERSON (BADOC MPS).

This pertains to the missing person identified as Gregorio Inez y Bermus, 47 years old, single,
person with disability (Mongoloid), and a resident of Brgy. Caraitan, Badoc, Ilocos Norte which was
reported to the Badoc MPS at about 1:00 PM of December 25, 2017 by his relatives. In this regard,
be informed that at about 6:00 PM of same date, afore-named missing person was found/located at
Nueva Era, Ilocos Norte by the personnel of Nueva Era MPS (PO3 Roel S. Sacalamitao, PO2
Dexter Dumalag, PO2 Ruben L Guillermo Jr., and PO1 Romeo G. Pacupac Jr.). The living body of
the said missing person has already been turned-over to his relatives in Badoc, Ilocos Norte. For
reference.

SPOT REPORT RE VEHICULAR TRAFFIC INCIDENT

At about 10:40 PM, of December 25, 2017, a report reached this office regarding a self-vehicular
traffic incident transpired along the national highway vicinity of Brgy. Poblacion #4, Pasuquin,
Ilocos Norte. Personnel of this office together with BFP, and RHU personnel responded to the
reported place of incident and learned that a color red Motorstar 125 motorcycle, no plate attached,
driven by Christian Lagazo, 16 years old, single, allegedly under the influence of liquor, and a res.
of Brgy. Sulbec this municipality, while heading north direction along the national highway vicinity of
Brgy. Pob. #4 this municipality the driver incidentally bumped the barricade situated at the right
shoulder of the road. The driver was brought to RHU Pasuquin and subsequently brought to
GRASMH Laoag City for further medication. The driver sustained abrasions on the different parts of
his body and face, while the motorcycle incurred broken head light assembly, and misaligned front
fender with still undetermined cost of repair.
