Medical Procedure

When: 1- 2 weeks before the start of the class

1st set of Medical: Any accredited Medical & Laboratory Clinic

Estimated Cost: Php. 2,000
1. Chest X-Ray
2. ECG,
3. CBC
4. Urinalysis
5. Puretone Audiometry (In Cebu, they can proceed to Manila Hearing Aid -2nd floor SM Cebu Tel# 231-8445)

Note: All results must have the complete name, signature and license number of the specialist who undergo the test.

2nd set of Medical: Civil Aviation Authority of the Philippines (CAAP), Manila (Medical Clinic)
When: Within 15 days after the 1st set of Medical exms
Estimated cost: Php 1,500

1. Eye Test
2. Drug Test
3. Dental Test

Requirements to bring and things to do for Medical in CAAP Manila (1 day)

1. 1st set medical result (including the film of chest X-Ray)
2. 2 (2x2) white formal background picture
3. Need to wear with collar polo shirt ( any color and design) no slipper & short pants
4. Eye glasses if needed
5. Need at least 8 hours rest (sleep) before the medical test
6. Need to get an early arrival (before 8am) in Manila and returning flight in the evening ( any preferred time)
