The Manufacturers Life Insurance Co. (Phils.), Inc. (herein referred to by ‘‘We’’, ‘‘Us’’, or ‘‘Our’’) hereby agrees to
pay the Benefits to the Beneficiary upon receipt of satisfactory proof of death of such persons (herein referred to by,
‘‘You’’, ‘‘Your’’ or ‘‘the Debtor’’) as shall be nominated from time to time by ‘‘Creditor’’ and with respect to
whom ‘‘Creditor’’ shall have remitted premiums, subject to all conditions and provisions of the Master Policy.

The maximum amount of insurance under the Policy as evidenced by this and other similar certificates shall not
exceed Php500,000.00.

The aggregate premium charges to You by the Creditor may not exceed the aggregate premiums charged by Us.

Upon receipt of satisfactory proof of death, We shall pay to the Creditor the following:

BENEFITS:

DEATH BENEFIT (Sum Insured: Php100,000.00)

Outstanding credit card balance, subject to the maximum Sum Insured, is payable to the Creditor. The balance of the
Sum Insured over the outstanding credit card balance is paid to Your designated beneficiary.

ACCIDENTAL DEATH & DISMEMBERMENT BENEFIT (Sum Insured: Php100,000)

The amount specified in the Schedule for such loss and limited to the Sum Insured applicable to the Debtor is
payable to the Creditor to cover the outstanding credit card balance. The balance of the Sum Insured over the
outstanding credit card balance is paid to Your designated beneficiary.

Payment by Us to the Creditor shall completely discharge under the provision with respect to the amount paid.

The Policy provides that any death benefits paid thereunder shall be first applied by the Creditor towards the
payment of Your Indebtedness to the Creditor. Any amounts in excess of Your Indebtedness shall be paid to Your
designated beneficiary.
