IMPORTANT INFORMATION’S & INSTRUCTION

Some important changes have been made regarding entrance test, carefully read them and prospectus for section of
“ELIGIBILITY CRITERIA” AND THE DOCUMENT REQUIRED of the category, applicable to you.
1. Follow the check list of “document required”.
2. One set of attested photocopies must be attached in given sequence.
3. Do not submit the original documents.
4. The application form and related documents should be deposited in a sealed envelope at Admission Cell, JSMU.
5. Carefully read the schedule and fee structure in the prospectus and daily visit the JSMU website till the admission
procedure is finalized.
6. All queries should be sent on email strictly, no personnel contact should be made.
7. Caution Note: if cash payment of any amount is made for admission, by anybody to anyone inside or outside JSMU, it
will be considered as Breach of law, liable to be penalized.
8. Keep photocopy of this form.
