I am giving my consent to answer the questionnaire given by Marvic Joshua A. Villaverde and Rhoby
F. Villanueva, Grade 12 students of Manuel S. Enverga University Foundation. I understand that I’m
going to be part of the research “Study of How Student Athletes Balance Their Lifestyle, Studies,
And Training”.

I am giving my time to answer all the question’s and give the answers they needed for their research. I
understand that I have been selected randomly to give my own perception about the topic.

“Study of How Student Athletes Balance Their Lifestyle, Studies, And Training”

As a teenager and a senior high school student, we know the importance of sleep on our life. It may
have a great impact to our emotions and health but we may also know your experience of your sleep
patterns and your sleeping habits and also get your answers to help us get your thoughts about sleep
patterns. Your answers may help us to make our conclusions to our research be answered by sharing
your experiences.
