STATEMENT OF RESPONSIBILITY

I, the president of _________________________ hereby certified my duties and responsibilities of my

club/organization. As the president, I am responsible with the overall responsibility over the officers and
members; Shall exercise leadership, prudence, and integrity in directing the club towards sustained
progress during his/her term; Establish goals and objectives that may guide and direct the activities of the
club and the means to attain the same as well as the mechanism for monitoring the club’s performance;
Ensure that the club complies with all relevant laws, regulations, and best practices and policies of the
school; Review the narrative report prepared by the Secretary before its submission to the OSA and JAT.

I, the treasurer of ________________________hereby certified my duties and responsibilities of my

club/organization. As the treasurer, I am responsible and accountable over the funds entrusted to
him/her. Prepare budget proposals for certain activities that the club may conduct. Prepare liquidation
reports for activities conducted by the club which are to be submitted within two weeks after every
activity; Ensure that every transaction involving the use of club’s fund has a reasonable purpose and
evidenced by the appropriate supporting documents; Serve as the model of the treasurer of other
recognized clubs/organizations/programs of the school; Avoid unauthorized expenditures or uses of the
club’s fund.
