CHATTEL MORTGAGE

KNOW ALL MEN BY THESE PRESENTS:

This Chattel Mortgage, made by and between:

____________, and ________________, Filipinos, both of legal

ages, residing at _______________Novaliches, Quezon City,
hereinafter referred to as the DEBTOR/MORTGAGOR;
- and -
__________, Filipino, of legal age, and a resident of
________________Mapulang Lupa, Valenzuela City, hereinafter
referred to as the CREDITOR/MORTGAGEE;

WITNESSETH; That:

The MORTGAGOR is indebted unto the MORTGAGEE in the sum of ONE

MILLION SIX HUNDRED SEVENTY THREE THOUSAND NINE
HUNDRED TWO PESOS (P P1,673,902), Philippine Currency, receipt of
which is acknowledged by the MORTGAGOR upon the signing of this
instrument, payable to the order of the MORTGAGEE without need of prior
demand or notice. The amount of FIVE HUNDRED THOUSAND PESOS
(P500,000) shall be payable on June 4, 2018, while the balance of ONE
MILLION ONE HUNDRED SEVENTY THREE THOUSAND NINE
HUNDRED TWO (P1, 173, 902) shall be payable on July 4, 2018, with
interest of 4.3% per month of delay until the loan under this Promissory
Note is fully paid.

In the event that the DEBTOR should fail to pay the amount in full including
the monthly interest at maturity, the maturity date for the entire loan being
on July 4, 2018, the CREDITOR/MORTGAGEE, _________ or her heirs and
assigns, may immediately foreclose this MORTGAGE over the motor vehicle
described under Official Receipt No. ___________ and Cr. No.
_____________of the Land Transportation Office, Quezon City, in the name
of ________________.

NOW, THEREFORE, for and consideration of the afore-mentioned

indebtedness, and to assure the performance of said obligation to pay, the
MORTGAGOR hereby conveys by way of CHATTEL MORTGAGE unto the
MORTGAGEE, her heirs, successors and as signs, the following personal
property now in the possession of said MORTGAGOR:

That it is the condition of this Mortgage that should the MORTGAGOR

perform the obligation to pay the afore-cited indebtedness of ONE MILLION
SIX HUNDRED SEVENTY THREE THOUSAND NINE HUNDRED TWO
PESOS (P P1,673,902) together with accrued interest thereon, within the
agreed term, this Chattel Mortgage shall be discharged and shall at once
become null and void and of no effect whatsoever, otherwise, it shall subsist
and remain in full force and effect and be subject to foreclosure in the
manner and form prescribed by law.

severally swear that we executed the foregoing Chattel Mortgage in order to
secure the indebtedness therein cited and for no other purpose or purposes
contrary to law.
