PETITIONER, by counsel and unto this Honorable Court, most

respectfully state: That –

1. Petitioner is of legal age, Filipino citizen, [widower/widow] and

2. Petitioner was once married to Name of Spouse;

A certified copy of the Certificate of Marriage dated [date] is

herein.

3. Out of the marriage, __________________________ was born;

4. ______________________, for whom this petition for

guardianship is filed, is a minor, being only __ years old and __

5. Said minor is currently residing with Petitioner at

6. Said minor is, by law, entitled to inherit from her deceased
