Dear Dr. Shuffett:
This letter is to provide information to the Board regarding the above-noted
Physician in support of his request to have his Agreed Order, which was filed of
record on August 3, 2018, modified to allow him to retum to the practice of
clinical medicine.
Dr. Heilig was referred to the Foundation due to concems of impairment in the
workplace. He met with me in my office on May 14, 2018, and based on the
information he shared over the course of our interview, I directed him to undergo
@ 96-hour inpatient evaluation at a Foundation-approved facility.
He underwent his evaluation at Hazelden on June 3, 2018, and was subsequently
diagnosed with Ambien Use Disorder Mild and Percocet Use Disorder Mild. The
evaluating team recommended documented abstinence from all substances of
abuse, including alcohol, individual therapy, and an evaluation with an addiction
psychiatrists.
He was also directed to take a prescribing course prior to his return to clinical
practice, which he completed at the Center for Professional Health at Vanderbilt
University in mid-August.
Dr. Heilig met with me and my staff on June 26, 2018, and signed a five-year
letter contract, which was developed based on the recommendations included in
his evaluation at Hazelden,
