1.1 KICK-OFF MEETING

1. Kick-Off meeting is required at starting up of contract which shall normally


be held at O&M Department. The Engineer shall set the date and time of
the meeting and shall notify the Contractor accordingly.
2. Kick-Off meeting shall be attended by the Engineer and his
representatives and the Contractor and his representatives.
3. The agenda for the meeting shall be provided by the Engineer a minimum
of three (3) working days prior to the meeting.
Topics for discussion will include but not necessarily be limited to the
following:
(a) Mobilization
(b) Start-up plan and schedule
(c) Start-up and validation prerequisites
(d) Quality Control and Assurance
(e) Possession of the site
(f) Engineer’s site facilities
(g) Contractor’ s site facilities
(h) Permit and Security procedures / traffic permits.

Drawing/Procedures and communication topics of discussion will include, but


not necessarily be limited to, the following:

(e) Monthly reports
(f) Method statements/procedures
(g) Progress meetings
(h) Material approvals

1.2 PROGRESS MEETINGS

1. Progress meetings shall be held monthly. The time and location of the
progress meetings shall be as directed by the Engineer. The Engineer
shall chair these meetings.
2. The Engineer shall prepare the agenda and submit it to the Contractor a
minimum of three (3) working days prior to the meeting.
3. Progress Meetings shall be attended by the Engineer and the Contractor
including respective representatives. Sub-contractors may attend as and
when required and when involved in the Matters to be discussed or
resolved but only when requested by the Engineer. In addition to the
attendees named herewith, Government departments and authorities and
any other regulatory agencies having jurisdiction over the Project, if
required, and such other persons the Engineer may designate may also
attend the meetings.
4. The Contractor shall submit the information itemized below to the Engineer
on each progress meeting:
(a) A list of activities (Work Order = WO's)

(b) A list of current activities (WO's) with an estimate of time required for
completion
(c) A list of changes to planned activities starting dates and durations
(d) A list of all outstanding activities
(e) A list of activities planned to start in the next period
(f) Any other information required by the Engineer’s Representative.

1.3 POST MEETINGS

1. Post meetings can be held on requisition bases on both parties. The time
and location of the post meetings shall be as directed by the Engineer. The
Engineer shall chair these meetings.
2. The requester shall prepare the agenda and submit it minimum of three (3)
working days prior to the meeting.

1.4 RECORD OF MEETINGS

1. The Engineer will record minutes of each meeting.


2. A copy of the minutes shall be furnished to the Contractor within four (4)
working days.
3. If the Contractor has any objections to the minutes he shall submit them in
writing to the Engineer not more than two (2) days after they are presented
to him. In the absence of any Objection, it shall be understood and agreed
that the Contractor accepts the minutes as true and complete record of the
meeting.

13. APPLICATIONS AND INVOICES

2.1 SUBMITTAL PROCEDURES


2.1.1 General
1. The Contractor shall submit to the Engineer for his review and approval all
procedures and data as called for in the contract Documentation or
subsequently as covered by variations.
2. Submittals and their contents shall be properly prepared, identified, and
transmitted in accordance with the Project Documentation or as the
Engineer may otherwise direct. Where possible and practicable, the
Contractor shall present submittals on A4 size paper.
3. All submittals shall be in triplicate unless otherwise specified in the Project
Documentation.
2.1.2 Schedule of Submittals
1. The Contractor shall prepare and submit a list of all submittals, required by
the Contract or otherwise, showing the forecast date for submission of
each item within five (5) days from the start of the Contract.
2. Procedures and data shall be submitted to meet the time schedules
stipulated in the Project Documentation and where such time schedules
are not specified, to meet the requirements of the approved programme.
The Contractor shall not allow less than fourteen (14) days for the review
of submittals by the Engineer (excluding the time necessary for delivery or
postal) and shall cause no delay in the work or the work of any other
Contractor.
3. Extension of time will not be granted because of the Contractor's failure to
make timely and correctly prepared and presented submittals with
allowance for the checking and review periods.

2.1.3 Method of Submittal


1. The Contractor shall deliver submittals by means of dated, signed, and
sequence numbered transmittals. The transmittal shall be on the
Contractor's letterhead and shall fully describe the submittal contents.
Submittals are not acceptable from sub-contractors, suppliers, or
manufacturers.
2. In each transmittal the Contractor shall state the Document numbers and
Specification Sections, Parts, and Paragraphs to which the submittal
pertains.

2.1.4 Contractor's Review and Engineer Approval


1. Every submittal shall bear the Contractor's review and approval stamp
certifying that the Contractor has:

 Reviewed, checked, and approved the submittal


2. Co-ordinated the contents with the requirements of the Works, the contract
Documentation and any related work
3. The Contractor shall make all required corrections and shall resubmit
corrected submittals until approved.
4. The Contractor shall direct specific attention in writing to revisions other
than the corrections Called for on previous submittals.
5. The Contractor shall identify each re-submittal with the sequel number of
the original submittal followed by consecutive number starting with "0" for
first submittal, "1" for second re-submittal, "2" for third re-submittal, etc.
6. The approval of submittals shall not relieve the Contractor of responsibility
for any deviation from the requirements of the Project Documentation
unless otherwise agreed with, and confirmed in writing by, the Engineer.
7. Any approval from the Engineer shall not relieve the Contractor of
responsibility for errors or omissions in the submittals.
8. Submittals which are incomplete, including those not correctly transmitted,
not correctly titled and identified, or not bearing the Contractor's review
and approval stamp, may be returned to the Contractor without review.

2.2 PROGRAMME
2.2.1 Programme of Works
1. The Contractor shall prepare and submit to the Engineer a Programme of
Works.
2. When instructed by the Engineer, the Contractor shall prepare and submit
a detailed programme for a particular section of the Works. The detailed
programme shall be an elaboration of the overall programme and shall be
in an appropriate form.
3. The Contractor shall incorporate the times of occupation of all sub-
contractors in the programme and shall make provisions for the access of
other Contractors as required by the Engineer.
2.3 ORGANIZATION CHARTS

Staff Organization Chart:


1. The Contractor shall submit a Staff Organization Chart for the Engineer’s
approval within ten (10) days of the start of the Contract.
2. The chart shall be as detailed in "Quality Assurance" Section.
2.4 PLANS:
2.4.1 Health and Safety Plan
1. The Contractor shall submit his Health and Safety Plan to the Engineer no
later than ten (10) days of the start of the Contract.

2. The chart shall be as detailed in Appendix - Safety.

2.4.2 Quality Assurance Plan


1. The Contractor shall submit his Quality Assurance Plan to the Engineer no
later than twenty (20) days of the start of the Contract.
2. The chart shall meet the requirements of clause of Quality Assurance.

2.5 REPORTS:
2.5.1 Technical Reports:
1. Where relevant, reports should be referenced to the Programme of
Works / QA Plan / ITP etc. (Detail refer to QA Section). To be submitted by
contractor on time.
2. Laboratory Report and any extra report (incase) if requested by Engineer
by written, should be submitted without any extra charge(s) (refer to
Operation Section)
3. TPA certification i.e.; Crane SWL, NDT etc. to be submitted by contractor
on time.
4. The Contractor shall submit one copy of each report unless otherwise
specified.
2.5.2 Progress Report:
1. The Contractor shall prepare and submit a monthly Report throughout the
duration of the Contract. The report shall be submitted to the Engineer
before the third day of each month. The report shall include the following:
 Manpower List
 Man hour List
 Material list that contractor used and changed during
construction/repair Period and/or reporting stage
 The date of commencement and completion of WO's
 Status of RO's / Permits and/or Municipalities clearance certificates.
 Details of problem areas
 Accidents to anyone working on the Site which required medical
attention
 Details of monthly progress and status of the Works
 Details of current or anticipated causes of delay along with their
estimated impact on progress and the corrective measures taken or
proposed.
 Status of Completion Certificates and Maintenance Certificates (MC).
 Photographs (if required).

2.6 MATERIALS APPROVALS:


Submittals shall be approved before the materials or equipment covered by
the submittals are shipped out in the case of foreign supplies and delivered to
the Site in the case of local supplies.

2.7 SAMPLES
Samples Definition: Samples are physical examples illustrating materials,
equipment, or workmanship and shall be used to establish standards by which
the work will be judged.
1. Each submittal shall include two sets of samples. One set of approved
samples and all disapproved samples will be returned to the Contractor.
2. Samples shall be taken in accordance with the provisions of relevant
standards where appropriate.
3. Samples shall be labeled and properly identified with the following:
(a) Date
(b) Part(s) of the Works for which offered
(c) Specification Section, Part and applicable Paragraph numbers
(d) Supplier/manufacturer
(e) Product identification (trade name).

2.8 CERTIFICATES
1. When stated in the Contract Documentation or requested by the
Engineer in writing, the Contractor shall submit manufacturer’s
certificates indicating that test results, component manufacture or
installation complies with the requirements of the Contract
Documentation.
2. A statement from the Contractor, sub-contractor, equipment supplier, or
agent indicating the product installation complies with the requirements
of the Contract Documentation shall not be considered as a certificate.

2.9 GUARANTEES AND WARRANTIES


Manufacturer’s Guarantees and Warrantees shall be submitted prior to
handover and final acceptance.

2.10 METHOD STATEMENTS / PROCEDURES / INSPECTION AND TEST PLAN


(ITP) / OPERATION AND MAINTENANCE MANUAL / DRAWINGS
(WORKING DRAWINGS AND SHOP DRAWINGS) ETC.
1. The Contractor shall prepare and submit Method statements /
Procedures / ITP for all activities.He shall also prepare and submit method
statement for specific work activities if it is a requirement of the Contract
Documentation or if instructed to do so by the Engineer.
2. The Method statements/procedures/ITP shall incorporate any supporting
calculations, drawings and manufacturer’s guidelines.

3. All the required Procedures as per QA Section are subjected for submittal
under this section requirement.
4. Each submitted drawing shall be complete with respect to dimensions,
design criteria, materials, connections, bases, foundations, anchors, and
the like, and shall be accompanied by technical and performance data and
by calculations as necessary to support the Information shown on the
drawings.

2.11 INTERNAL/EXTERNAL AUDIT


Internal/External audit results/reports to be submitted for Engineer review by
contractor, detailed referred to audit section under Clause 7.1.

2.12 APPLICATIONS AND INVOICES


Applications and invoices on monthly basis shall be submitted in accordance
with the Payment conditions of Contract. For details refer to payment section.

DEPARTMENT OF ASSETS AFFAIRS RELATED PROCESSES:
INSPECTING AND TESTING

3.1 CONTRACTOR’S RESPONSIBILITY


1. The Contractor shall be responsible for the development, implementation
and maintenance of a Quality "Assurance Programme" consistent with the
requirements of this Section (but not limited to), that assures
workmanship, activities, equipments and material conformance to the
requirements of this scope of work and requirements of applicable
references of this scope of work.

2. The contractor should preferably hold a valid ISO Management System


Certificate during the entire period of this scope of work.

3. The Quality Assurance Programme shall assure the maintenance of


adequate quality throughout all applicable areas of Contract performance.

4. The Quality Assurance Programme shall provide a mode of operation that


emphasizes the identification, correction, and prevention of deficiencies
and discrepancies.

5. Procedures to implement the Quality Assurance Programme shall be


developed. Specific responsibilities shall be assigned, and individual job
authorities clearly delineated.

6. Procedures shall be established to systematically verify compliance with


the requirements of this Part and any other requirements of the
Specification necessary to show conformance with the contract
Documentation.

7. The Contractor's Quality Assurance Programme shall be subject to


evaluation by the Engineer to determine where the Quality Assurance
Programme meets contractual requirements. Audits of Contractor, sub-
contractor, and Contractor activities will be performed by the Engineer to
verify compliance with the Specifications and all referenced standards.
Audits will be performed on a systematic basis or as warranted by general
quality trends. The Contractor shall provide all necessary assistance to the
Engineer.

8. The Contractor shall impose all quality assurance requirements upon sub-
contractors, suppliers, manufacturers and any other parties associated
with the Contractor who are involved in the Project.

Quality Assurance Programme shall comply with ISO Management Systems


requirements as following but not limited to;

3.2 DOCUMENTATION

3.2.1 Control of Documents


General:
A. The Contractor shall establish and maintain documented procedures to
control all documents and data that relate to the requirements of the
Specification. Documents and data can be in the form of any type of
media, such as hard copy or electronic media.
B. Detail, Method of submittal must refer to Submittal section of this scope of
tender document,
Document and Data Approval and Issue
C. The documents and data shall be reviewed and approved for adequacy by
authorized personnel prior to issue. A master list or equivalent document
control procedure identifying the current revision status of documents shall
be established and be readily available to preclude the use of invalid
and/or obsolete documents.

This control shall ensure that:

(a)The pertinent issues of appropriate documents are available at all


locations where operations essential to the effective functioning of the
quality system are performed.
(b)Invalid and/or obsolete documents are promptly removed from all points
of issue or use, or otherwise assured against unintended use

(c)Any obsolete documents retained for legal and/or knowledge


preservation purposes are suitably identified.

Document and Data Changes


D. Where practicable, the nature of the change shall be identified in the
document or appropriate attachments

3.2.2 Control of Quality Records


i. The Contractor shall establish and maintain documented procedures for
identification, Collection, indexing, access, filing, storage, maintenance
and disposition of quality records.
ii. All quality records shall be legible and shall be stored and retained in such
a way that they are readily identifiable and retrievable in facilities that
provide a suitable environment to prevent damage or deterioration and to
prevent loss.
iii. The Contractor shall supplement these quality records as necessary to
monitor quality throughout the Contract period.
iv. Quality records shall be maintained to demonstrate conformance of
materials/ Construction activities etc. to specified technical requirements
and the effective operation of the quality system.

3.3 CONTRACTOR'S MANAGEMENT RESPONSIBILITY


Contractors QA Programme shall clearly state the above mentioned subject
as below discipline:
i. Management Commitment in order to ensure of development and
implementation of quality management system.
ii. Customer focus
iii. Quality Planning
iv. Responsibility, authority and communication (as detailed on Organization
Plan)

3.4 RESOURCE MANAGEMENT


i. QA Programme of contractor must clearly state this subject,
that; all the determined and needed resources as per this contract
documentation shall be provided by contractor in order to meet
requirements on; Human resources, Infrastructure and work environment,
which to be checked the details on BOQ and other sections of this scope
of work.
ii. Contractor is responsible to safely care of all the asset
handed over to him under this contract Document.

3.5 OPERATION AND MAINTENANCE REALIZATION


A) Any part of the works must not interfere and/or interrupt the day to day
operations and maintenance activity of the Pumping Stations or any other
related operations within Drainage operations and maintenance.

3.5.1 Quality Assurance Plan

i. The Contractor shall submit a Quality Assurance Plan to


the Engineer for approval no later than twenty (20) days from awarding of
the Contract.

ii. The Quality Assurance Plan shall describe all of the


Contractor’s quality assurance procedures that will be used throughout the
Contract and shall include but not be limited to the following:

(a) a detailed description of procedures, instructions, and reports to be


used to ensure compliance with the Contract Documentation
(b) a detailed description of procedures for reviewing shop drawings,
samples, certificates, and other submittals necessary for compliance
with the Contract Documentation
(c) a detailed description of procedures used to identify, report and resolve
problems
(d) a description of the services provided by outside organizations such as
testing laboratories, approved workshop(s), Consulting, TPA and etc.
(e) a detailed description of inspections and tests required
(f) a test and inspection schedule keyed to the construction / repair
(g) copies of forms and reports to be used to document quality assurance
operations
(h) the names of personnel responsible for each part of the Works
(i) a submittal status log listing required submittals and action required by
the Contractor and Engineer
(j) a detailed description of document and submittal control procedures
(k) procedures to identify and control use of items and materials

iii. No requests for payment from the Contractor shall be


processed until the Contractor’s Quality Assurance Plan gets approved.

3.5.2 Organization Plan


i. The Contractor shall submit an Organization Plan to the Engineer for
approval not later than (10) days from the start of the Contract.
Note: Overall organization chart for this project must be submitted by
tenderer along tender document.

ii. The Organization Plan shall provide the name, qualifications, and
experience of key staff.
Note: Manpower qualification requirements are detailed on Qualification
section.

iii. The Organization Plan shall show the organization of Contractor’s team
and shall include, but not be limited to, the following:

(a) indicate and describe the area of responsibility and authority of each
individual in the team.
(b) identify the Quality Assurance Representative / Safety officer showing
that the position is independent of the job supervisory staff with clear
lines of authority
(c) an organization chart identifying communications.

iv. CONTRACTOR’S Quality Assurance Representative /


Inspector Qualification
1) The Contractor shall notify the Engineer in writing prior to re-assigning
or replacement of any of the quality assurance personnel designated in
the Quality Assurance Organization Plan.
2) The Contractor's quality assurance personnel shall have the authority
to stop any portion of the work which does not comply with the
requirements of the contract Documentation.
3) Personnel inspecting and testing special operations (e.g., Material Lab,
welding, brazing, etc.) shall have the experience, training, and
certification commensurate with the scope, complexity, or nature of the
activity. Such personnel shall be approved by the Engineer.
4) The Contractor shall submit the following for Quality Assurance
representative inspection and test personnel:
(a) qualifications description
(b) skill evaluation
(c) certification credentials.

3.5.3 Inspection and Test Plan (ITP)


i) The Contractor shall submit an Inspection and Test Plan to the
Engineer for approval not later than 15 days from the start of the Contract.
ii) The Inspection and Test Plan shall consider the requirements of each
Section of the Specification in turn and shall identify the following:
(a) all required inspections and tests required as per of this Specification
(b) the required testing frequency
(c) the acceptance/rejection criteria
(d) details of other agencies having jurisdictional authority over the work.
iii) Each inspection and test shall have a unique test reference number.
iv) No work covered by the Inspection and Test Plan shall begin until the
plan has been approved.
3.6 DRAINAGE NETWORKS OPERATION AND MAINTENANCE DEPARTMENT
OF ASSETS AFFAIRS RELATED PROCESSES

3.6.1 General
i) QA Programme of Contractor shall clearly state; Determination of
requirements related to O&M Services, Review of requirements related to
the O&M Service, and client communication.

ii) All listed references at "Reference Section" shall be considered as


regulatory requirements related to operation and Maintenance Service by
contractor.

4. INSPECTION AND TESTING

4.1 General
)i The Contractor shall provide equipment, instruments, qualified
personnel, and facilities necessary to inspect the work and perform the
tests required by the contract Documentation.
)ii The Contractor shall carry out all inspection and testing as per the
inspection and testing Section requirements under clause 3.8.

4.2 Inspections and Tests


i) All inspections and tests shall be conducted in accordance with written
test procedures as detailed in the Quality Assurance Plan and Inspection
and Test Plan that have been reviewed and approved by the Engineer.
ii) Inspection and test procedures shall be submitted for approval shall
include, but not be limited to, the following:
(a) Inspection/test procedure reference
(b) References to Clauses of this Specification and other standards along
with applicable inspection/test levels specified therein
(c) Prerequisites for the given inspection/test
(d) Required tools, equipment
(e) Necessary environmental conditions
(f) Acceptance criteria
(g) Data to be recorded
(h) Test results reporting forms
(i) Identification of items tested.

iii) Approved procedures and instructions shall be readily available and


used by inspection and Test personnel at the time of inspection or test. All
revisions to these procedures and instructions shall be approved prior to
being used to inspect or test the work. No deviations from the approved
procedures and instructions shall be allowed without written authorization
from the Engineer.
iv) The Contractor shall furnish the Engineer with a signed and stamped
inspection report for each item of work to be inspected. The report shall
indicate whether the item of work, material and/or equipment complies with
all the inspection/test criteria, within max 2 days after test.
v) Inspection and test reports shall, as a minimum, identify the following:
(a) inspection/test procedure reference
(b) name of inspector/tester
(c) observations/comments
(d) specified requirements
(e) acceptability
(f) deviations/non-conformance (if any)
(g) corrective action (if any)
(h) evaluation of results
(i) signature of authorized evaluator.

vi) The Contractor shall clearly document and identify the inspections and
test status of material or equipment throughout maintenance. Identification
may be by means of stamps, tags, or other control devices attached to, or
accompanying, the material or equipment.

5. MATERIALS
Handling and Storage of Materials
.1 The Contractor shall establish procedures for handling and
storage of materials and equipment.
.2 The Contractor’ s storage and handling procedures shall be
designed to prevent damage, deterioration, distortion of shape or
dimension, loss, degradation, loss of identification, or substitution.
.3 The handling procedures shall address the use, inspection and
maintenance of special devices such as crates, boxes, containers,
dividers, slings, material handling and transportation equipment and other
facilities.
.4 The Contractor shall identify equipment and/or material
requiring special handling or storage.
.5 Lifting Equipment of Contractor must be safe and meet the
requirements International Lifting Equipment SWL.

6. CONTROL OF MONITORING AND MEASURING DEVICES


The Contractor shall establish and maintain documented procedures which
conform to accepted and approved national or international standards to
control, calibrate and maintain inspection, measuring and test equipment used
by the Contractor to demonstrate the conformance of materials, equipment
and Operation and Maintenance work with the requirements of the contract
Documentation.
1. Inspection, measuring and test equipment shall be used in a manner
which ensures that the measurement uncertainty is known and is
consistent with the required measurement capability.
2. The Contractor shall establish a unique identification number for each
item of measuring and test equipment. This unique identification number
shall be permanently affixed to each item of measuring and test
equipment.
3. The Contractor shall ensure that each item of inspection, measuring
and test equipment is calibrated at intervals as per O&M Dept. Calibration
procedure (QP4.4) requirements. Valid calibration certificates for
measuring and testing equipment shall be present and available for
inspection during inspections and tests.
4. The Contractor shall establish a log of all measuring and test
equipment and record:
(a) equipment description
(b) identification number
(c) date of the last calibration
(d) date that the next calibration is due.

7. MEASUREMENT, ANALYSIS and IMPROVEMENT


QA Programme of Contractor shall state plan and implement the monitoring,
Measurement, analysis and improvement processes needed to this scope of work
requirements as following points but not limited to;

7.1 QUALITY AUDITS:


7.1.1 Contractor’s Quality Audit
i) The Contractor shall establish and maintain documented procedures for
planning and implementing internal quality audits to verify whether quality
activities and related results comply with planned arrangements and to
determine the effectiveness of the quality system.
ii) Internal quality audits shall be scheduled on the basis of the status and
importance of the activity to be audited and shall be carried out by
personnel independent of those having direct responsibility for the activity
being audited. Unless otherwise agreed with the Engineer in writing, the
Contractor shall carry out a quality audit on a regular basis. The frequency
must be agreed with the Engineer.
iii) The results of the audits shall be recorded and brought to the attention of
the personnel having responsibility in the area audited. The management
personnel responsible for the area shall take timely corrective action on
deficiencies found during the audit.
iv) Follow-up audit activities shall verify and record the implementation and
effectiveness of the corrective action taken.
v) The results of the Contractor’s quality audits shall be submitted for review
by the Engineer. The Contractor shall implement any recommendations
made by the Engineer based on the results of the internal audit.
vi) The Contractor shall allow the Engineer to observe the Contractor’s
internal audit upon request.

7.1.2 Engineer’s Quality Audit


i) The Engineer will take quality audit of any of the Contractor’s activities at
any time and/or planed during the course of the Contract. The Contractor
shall make all personnel and facilities available to the Engineer as
necessary to undertake quality audits.
ii) The Engineer shall make the results of his quality audit available to the
Contractor for information. The Contractor shall implement any
recommendations made by the Engineer based on the results of the
Engineer’s quality audit.

7.2 NON-CONFORMANCES MONITORING AND IMPROVEMNET

i) The Contractor shall establish and maintain documented procedures


(refers to O&M Dept. NCR Procedure) to ensure that an item that does not
conform to the requirements of the Contract Documents is prevented from
unintended use or installation. This control shall provide for identification,
documentation, evaluation, segregation (when practical) and disposition of
the non-conforming item(s).
ii) The monitoring system shall apply to material and equipment as well as
construction / Repair activity, which fail to conform to the contract
Documentation.

7.2.2 Review and Disposition of Non-conforming Items


i) The responsibility for review and authority for the disposition of non-
conforming items shall be defined in the Quality Assurance Plan.
ii) Non-conforming items shall be reviewed in accordance with documented
procedures. A non-conforming item may be:
(a) Reworked to meet the specified requirements
(b) Accepted with or without repair if agreed in writing by the Engineer
(c) Re-graded for alternative applications
(d) Rejected or scrapped.
iii) The proposed use or repair of an item which does not conform to the
requirements of the contract Documentation shall be reported to the
Engineer. The description of the non-conformity and of repairs shall be
recorded to denote the actual condition.
iv) Repaired and/or reworked of Operation and maintenance activities shall
be inspected in accordance with the Quality Assurance Plan and/or
documented procedures.
7.2.3 Identification of Non-conforming Items
The Contractor shall clearly identify each non-conforming item with a status
tag or other distinguishing mark. The Contractor shall establish procedures for
installing, monitoring, and removing these status tags and identify personnel
authorized to remove status tags.

7.2.4 Non-conformance Records


i) The Contractor shall provide the Engineer with the following information for
each non-conformance:
(a) Identification of non-conformance
(b) Description of non-conformance
(c) Evaluation of non-conformance to establish the cause
(d) Recommended corrective action
(e) Date non-conformance was identified
(f) Date corrective action was completed
(g) Description of final corrective action.

ii) The Contractor shall establish and maintain a non-conformance log. The
log shall contain the following information as a minimum:
(a) Sequential reference number
(b) Date issued

7.2.5 Corrective and Preventive Action


i) The Contractor shall establish and maintain documented procedures for
implementing corrective and preventive action.
ii) Any corrective or preventive action taken to eliminate the causes of actual
or potential non-conformities shall be to a degree (critical, Major, Minor)
appropriate to the magnitude of problems and commensurate with the
risks encountered.
iii) The Contractor shall implement and record any changes to the
documented procedures for implementing corrective and preventive
action.

7.2.6 Analysis of Data


Requirement for analysis of data to be stated on QA programme of contractor
as the above mentioned requirements.

8. INSPECTION AND TESTING ACTIVITIES:

8.1 Inspection
8.1.1 General
i) The Contractor shall provide equipment, instruments, qualified personnel,
and facilities necessary to inspect the work and perform the tests required
by the contract Documentation.
ii) The Contractor shall repeat tests and inspections after correcting non-
conforming work until all work complies with the requirements. All re-
testing and re-inspections shall be performed at no additional cost to the
Client.
iii) Inspections and tests conducted by persons or agencies other than the
Contractor shall not in any way relieve the Contractor of his responsibility
and obligation to meet all Specifications and referenced standards.

8.1.2 Inspections and Tests


i. All inspections and tests shall be conducted in accordance
with written test procedures as detailed in the "Quality Assurance Plan"
and "Inspection and Test Plan" that have been reviewed and approved by
the Engineer.
ii. Inspection and test procedures shall be submitted for approval
as detailed in QA Section.

iii. The Contractor shall furnish the Engineer with a signed and
stamped inspection report for each item of work to be inspected. The
report shall indicate whether the item of work, material and/or equipment
complies with all the inspection/test criteria, within max 2 days after test.
This must be supported by photographic evidence of tags installed.
iv. The Contractor shall clearly document and identify the
inspections and test status of activities.

8.1.3 Inspections by the Engineer during Maintenance Period


i. The Engineer will give the Contractor due notice of his intention to carry
out any inspections during the defects liability period.
ii. The Contractor shall arrange for a responsible representative to be
present at the times and dates named by the Engineer.
iii. The Contractor’s representative shall render all necessary assistance and
take note of all matters to which his attention is directed by the Engineer.

8.2 Civil Laboratory Material and Testing


Tests should be carried out by approved laboratories and reports to be
submitted to the Engineer for his review and approval.

All materials used in contact with sewage or its atmosphere must withstand
corrosion and satisfactory evidence of test result to be submitted (all as per
QCS Section 5 and 8 requirements).

8.3 Lifting Equipment


1. Requirements of this section are applicable for lifting equipment in site
or any temporary crane using by contractor.
2. Cranes shall be inspected and tested in accordance with the
requirements of Clause 54 BS 466 or BS 5744 and witnessed by the TPA
and/or Engineer. Chain Blocks/Hand cranes shall be similarly tested in
accordance with the requirements of BS 3243.
3. The works tests shall include Proof tests during which a 25% overload
shall be lifted by the hoist at the middle of the crane span and sustained
under full control whilst it is moved up and down at both normal and creep
hoist speeds. Whilst still under overload the crab unit of each crane shall
be operated from end to end of its travel.
4. The mechanism and controls for the long travel motions shall be tested
under light running conditions without moving the crane.
5. Tests on manually operated cranes and hoists shall comply with the
above clauses as they are applicable.
6. Control panels, motor and any factory installed wiring shall be
inspected as well.
7. All the using lifting equipment accessories as; slings, rope, hook etc.
which are called "Tackles" shall be safe to use with cranes and all of them
must be certified as below codes requirements by contractor via an
approved authorized TPA prior to use.

9. Third Party Agency (TPA)


1. TPA is the approved company or organization responsible on behalf of the
Engineer for inspection /service/release. In the all mandatory required
services of TPA, as mentioned in this scope of work the hired TPA shall be
paid by contractor.
2. Contractor shall hire approved TPA on the below categories, to fulfill this
scope of work requirements;
a. Laboratory for Civil Materials and compaction sampling and testing
b. TPA Certificates for regularly inspection and test of lifting equipment.
c. Calibration of Measuring Devices and certifying them shall be carried
out periodically by an approved calibration Laboratory.

10. Reports and Certifications


1. Daily, Weekly and Monthly status reports in accordance with WO's and are
to be submitted to the Engineer.

2. Laboratory reports for civil material compaction tests and etc. are to be
submitted to the Engineer.

3. Lifting Equipment testing certification is to be submitted to the Engineer.

4. Calibration of Measuring Devices certificates are to be submitted to the


Engineer.

5. Company External Audit (if any) report as periodically as obtained from the
Auditors to be submitted for Engineer information and review.

Asset Tagging Works

A) Introduction

Drainage Operation and Maintenance Department of Assets Affairs of Public Works


Authority (DOM), requires a Contractor to supply and install Asset Tags (Tags) on
pumping station assets in Qatar. Asset tagging supports effective tracking of O&M
activities on each and every asset; thereby aiding in the creation of a history of
repair, asset failure, and data collection. This information is used for asset planning
activities, maintenance planning and investment level life cycle costing throughout
the asset base. The benefits reaped from data collection, analysis and sharing allow
the Pumping Stations’ Engineers to make informed decisions on maintaining,
repairing and rehabilitating of the assets and track asset whereabouts; thereby,
making the O & M department more efficient.

This contract is for supply and install of barcoded asset tags (works). This contract
incorporates section C General Specifications, detailed technical specification of the
tags and includes the fixing and installation method of the tags. The scope is to
supply and install 3,000 asset tags.

Duration of this contract is for a twelve (10) month period.

B) Summary

1) The Asset Tagging contract is basically; "Supply and Fix and/or Install” of any
of the above mentioned required work's field, as per the DNO&MD
requisitions.

2) This contract is for supply and install of barcoded asset tags (works). This
contract incorporates section C General Specifications, detailed technical
specification of the tags and includes the fixing and installation method of the
tags. The scope is to supply and install 3,000 asset tags.

3) This contract is "on-call" basis and all the activities are re-measurable by
Engineer.

4) DNO&MD reserves right to supply any item from BOQ or any items listed/not
listed in this scope of work, from store which has to be collected by contractor,
with approval from the Engineer.

5) Duration of this contract is for 10 months.

C) Brief Description

1) The work shall be performed in accordance with the standards of the DOM.
2) The scope of works mainly are for the following disciplines:
a) Civil
b) Mechanical
c) Electrical

3) The Contractor shall supervise and attend to the work at all times to ensure
the quality of the work and the safety of all workers on site during
performance of the activities.
4) The Contractor shall use materials from DOM approved manufacturers and
suppliers. All required material’s testing shall be conducted by DOM
approved Materials Testing Laboratories.
5) The Contractor shall properly dispose of surplus materials (as defined by
DOM).

6) The Contractor shall comply with all Qatar Construction Standards (QCS),
and Public Works Authority (Ashghal/PWA) standards.

7) The Contractor shall return and deliver all salvage materials to designated
DOM stockyards.

8) The Contractor may not assign, or sublet, the works, or any part thereof, to
any subcontractor without prior written approval from the PWA designated
administrator so named in the award document.

9) The Contractor is required to propose and obtain the Engineer’s approval to


establish a dedicated stockyard for the safekeeping of the materials for this
works as per the list contain in the Minimum Stock Requirement bill of
quantities.

10)The Engineer reserves his right to supply any item from BOQ or any items
listed/not listed in this scope of work, from the Engineer’s store which has to
be collected by the Contractor.

1) Objective

This section provides guidance on the detailed technical specifications to enable the
procurement of high quality Asset Tags which meet Ashghal/PWA Tagging
requirements and standards. These specifications, requirements and standards
provide a basis for implementation of a tagging system of operational assets, within
the O & M drainage facilities.

A full list of operational assets (Asset Types) including Tag references have been
developed and will be made available to the bidders during the tender process.

Bar Coding overview


Barcode Technology
The characteristics of a linear barcode (1D) consist of a series of vertical, parallel,
adjacent bars and spaces. Barcode technology uses an identification process which
allows real-time data to be collected accurately using a barcode reader from either a
1D 2D or DPM barcode image. Ashghal proposes to implement the 1 D bar code
system.
A barcode reader uses a laser beam to read the image and translate the information
into digital data that can be sent it to a computer for storage or to engage another
process.

Typically the most common type of barcode used for labeling products and assets
are 1D Barcodes which, consist of a series of vertical, parallel, adjacent bars and

E) General Notes on Technical Specification

1) Contractor's staff accommodation and any other stores, offices shall be


arranged for the period of contract.
2) Contractor shall not delay the work without acceptable reason.

3) Any damages and/or disruption which is as a result of contractor’s poor


workmanship, mistakes and or supply of inadequate resources and
equipment, DNO&MD will repair/fix and rectify the problem and Contractor will
be billed for incurred cost.

Supply of Material by Contractor


1) Contractor to supply and deliver the stated quantity of pipes (as stated in the
Minimum Stock Requirement BOQ) with the corresponding sizes together with
the fittings, gaskets and the associated accessories as a MINIMUM STOCK at
all time in Engineer’s approved stock yard.

2) Contractor must ensure that required materials are available and deliver to site
to comply with the programme requirements.

3) Contractor is responsible and must ensure that materials are available at every
stage of the programme.

4) Contractor to price for each item as defined in the BOQ and technical
specification including costs of procurement, storage up to delivery to the
installation site.

5) All the material to be prepared from approved brand/manufacturer by Assets


Affairs, (see Annexure "B"), the latest list shall be obtained by contractor from
Engineer during procurement stage.

6) Tenderer must submit his sources/confirmations for the tender approved


brand/manufacturer or any other alternative materials on his technical
submission with this tender document.

7) DNO&MD reserves right to supply any item from BOQ or any items listed/not
listed in this scope of work, from store which has to be collected by contractor,
with approval of Engineer. The corresponding deduction will be made from the
contractor payment for the materials supplied by the Engineer.

8) Contractor has to procure required materials and arrange in his stores taking all
possible precautions against defects and damage. Any damage or defect
occurring to the materials shall be the sole responsibility of the Contractor and
he shall remain responsible for any damage to the materials supplied and
replace them with good quality.

9) Contractor has to store all materials in his stores under cover, clean and tidy as
per manufacturers’ recommendation, and transport to site safely whenever
necessary.

10)Any supplied material by contractor is subjected for inspection by engineer prior


to use in the site.

11) As soon as materials are delivered in his stores; Contractor has to notify the
Engineer officially to inspect them. Final receipt shall be acknowledged after
those materials having been inspected by the Engineer. Only after obtaining
Engineer’s approval, materials can to be used on site.
12)After inspection and final acceptance, the Contractor shall re-arrange the
supplied items in their final position in the store, as directed by the Engineer.
13)The Contractor shall be responsible for any defects that may appear in the
materials after final receipt.

14)The Engineer reserves the right to accept or reject the whole supplied materials
or part thereof, if after inspection, they are found to be incompatible with tender
specification and approved samples submitted by the CONTRACTOR. The
Engineer even can reject any materials when they have been brought to the site
for laying/installation, if found defective.

15)The CONTRACTOR shall replace those materials which are rejected by the
Engineer within a reasonable duration or as instructed by the Engineer to
safeguard the scope of work or without causing delay to any issued WO's.

16)Should supply of all materials or part thereof be unduly delayed beyond agreed
delivery date as specified herein above of this Contract, which also includes
replacement of rejected materials, then the Contractor must do his best to bring
them as soon as possible, without causing delay to any issued WO's.

17)At the end of contract, the Contractor shall deliver the complete quantity of
materials as stated in the MINIMUM STOCK REQUIREMENT BOQ to PWA
store or to a location as directed by the Engineer. Accepting of these materials
will be subjected to re-inspection at the site by the Engineer.

Note: Payment of these materials will be based on the rate under the Minimum
Stock Requirements related BOQ up to maximum of the quantity mentioned in
the Minimum Stock Requirements BOQ.

Permits
1) The Engineer is responsible to submit the permit for the work from different
Authorities except the Police Permission which will be the contractor's
responsibility.

2) Contractor has to prepare RO-1 for all types of service, and conduct site
survey for critical cases before raising RO-1 and gather data for the required
work and submit the same to engineer.

3) PWA will provide RO-2, before opening the ground for any purpose, the
Contractor must be fully aware of underground services existing in that
particular vicinity by reviewing concerned drawings and make trial pits, if
necessary.

Note: Detail as per DNO&MD permission procedure.

Emergency and Shut Down


1) Based on nature of works contractor is responsible to request for any required
shut down as per the DNO&MD shut down procedure. The contractor must
liaise closely with the Engineer to ensure that the works does not interfere with
the regular maintenance work.

Traffic control
1) The traffic control at road work to protect the public and the men working on
site while undertaking the works is the sole responsibility of the contractor.
The contractor is to make sure that traffic control devices are in place and
well maintained throughout the duration of the works to the satisfaction of
the traffic police.

2) The traffic signs must guide traffic clearly and safely during night or in
inclement weather through the length of the works to the satisfaction of the
traffic police.

3) The use of any methods of traffic control must be agreed with the traffic
police.

4) All excavations/obstructions on or near the foot way must be fenced off and
signed for pedestrian protection.

5) Each sign used by the contractor must have his identity clearly marked
together with telephone numbers which may be used in case of emergency.

Method and Discipline


1) The contractor will receive the work instruction at least one day before the
work, in some urgent cases, the work instruction will be on the same day,
and the contractor has to be ready to execute the work immediately.

2) The work will be on re-measurement basis. The contractor representative


and the engineer representative shall measure the work immediately after
completion, if the contractor representative fail to attend, the Engineer will

take the measurement and the Engineer measurement is final and shall be
used for payment purposes.

3) Contractor is responsible to supply and fix any facility required to deal with shut
down.

4) Contractor is responsible to manage and make available at any time the


minimum number of staff and machinery to cover any urgent work at any
time (24 hours, 7 days/week).
Note: For details, please refer to Method and Discipline clause, of this
tender document.

Safety
1) Contractor shall follow strictly the safety procedure while working, safety at
the site for the public and for the Contractor’s staff or the properties is the
sole responsibility of the Contractor.
Note: Detail of safety requirement is in Section 3 herein.

2) Contractor has to make available all the time all required Personnel
Protection Equipment (PPE) such as; calibrated gas monitor, safety
harnesses, access ladder, air blower, etc. on site. All work shall be under
monitoring of qualified safety officer of Contractor.

3) Contractor is responsible to arrange enough staff, including transportation to


and from the work sites, and all their requirements as per Government of
Qatar Laborers rules.

Reporting
Contractor is responsible to submit weekly report, monthly report / evaluation
and case by case report supported by high resolution photos for the work to
the Engineer.

The Contractor will be responsible to submit photographic evidence of


installed tags.

F) Materials Specification
General
a) Proposed material for supplying by contractor, are subjected for approval of
Engineer as per submittal procedure, prior to use/procurement by Contractor.

b) Material shall hold technical data sheets, test certificate and certificate of
originality, which must be traceable to the delivered material.

c) Supplied materials are subjected to inspection by Engineer to ensure of


conformity with approved submittal(s).

d) Material supplied by Contractor and their inspection and test requirements shall
meet requirements of this scope of work, Qatar Construction Specification
(QCS).

G) Method and Discipline of Works

1. The duration of the contract is 10 months.


2. Contractor has to mobilize sufficient team with facilities provision to handle
required jobs without affecting progress. Each team shall have adequate
personnel. Contractor must be prepared to mobilize more manpower if
demand for more tagging of assets arises.

3. Requirement for supplying material by contractor, to be referred to "General


Notes on Technical Specification"

4. WORK ORDER (WO)

4.1 There are two (2) types of WO form to be used during the execution of the
work namely,

Form No. 1 – Estimated WO: Contractor will be instructed using this Form
which contains the initial details and instructions of the work to be performed.

Form No. 2 – Actual WO: Upon completion of the works within the stipulated
and agreed requirements, Contractor will submit this Form to obtain the
Engineer approval and to finalise the said works for payment purposes.

4.2 The Contractor will be requested to execute any work through a WO (Form
No. 1) that describe the SCOPE OF WORK and TIME LIMIT of the entrusted
work depending on its nature and aligned with the overall programme of the
works. A specimen WO (Form No. 1) is attached in Section B.

Note: The WO (Form No. 1) will be clarified as Normal or Urgent by Engineer


at the issuing stage.

4.3 The contractor will receive the work instruction at least one day before the
work; in some urgent cases the work instruction will be in the same day, the
contractor has to be ready to execute the work immediately.

4.5 The Contractor shall render his scope/services through Work Order (WO's).
Each WO excluding or including reinstatement works (where and if applicable)
has to be completed within stipulated Time Limit as specified in the WO. The
Contractor has to agree with the Engineer on Time Limit against each WO in

days/weeks depending on its nature and site conditions aligned with the
programme of works.

5.On the normal cases, one calendar week is the maximum time to reply/agree about
the Time Limit from contractor side.

6.Contractor shall mobilize his resources immediately after the Contract Effective Date
and each day to attend as much as WO's as possible without affecting
progress.

7.The work will be on re-measurement basis, the contractor representative and the
Engineer representative shall measure the work immediately after completing
each phase of the work, if the contractor representative fail to attend the
measurement, the Engineer will take the measurement of his representative
as the approved/agreed one.

8. Contractor is responsible to supply and fix any required facility which is


necessary to deal with urgent works.

9. Contractor is responsible to manage and make available at any time the


minimum number of staff and machinery to cover any urgent work at any time
(24 hours, 7 days/week).

10. Contractor shall be prepared to undertake works in any of existing networks,


on any type of environment and in both urban and rural situations and equip
himself accordingly to meet all situations.

11. All drawings associated with this tender are appended in Section "E" and
Contractor has to approach the Engineer to obtain any other drawings or
details which are not enclosed but necessary to execute the work.

12. Should the Contractor require further drawings that may be necessary for the
execution of the WO he shall when instructed by the Engineer to prepare and
submit such detailed drawings with adequate notes in writing for the approval
of the Engineer. The Engineer may within a reasonable period approve,
amend or change these drawings or may require the Contractor to produce
amended drawings. No drawing shall be valid unless it is approved and signed
by the Engineer.

13. Contractor must obtain necessary permits/RO from the concerned authorities
to proceed with the work.

14. Particular care must be taken to avoid damage to all underground services
(Public Utilities and Services) during the execution of the works. Although the
Engineer will provide RO-2, before opening the ground for any purpose, the
Contractor must be fully aware of underground services existing in that
particular vicinity by reviewing concerned drawings and make trial pits, if
necessary. The Contractor shall be responsible for any damages caused to

such utility services due to his negligence and shall pay fully all charges and
keep the Engineer informed against all consequences and works.

15. Contractor must submit necessary site clearance certificate from the
Municipalities/concerned Authorities (if applicable) for the process of the
Completion Certificate.

16. On completion of any works, Contractor shall submit WO (Form No. 2) and
ensure the submission of all related As-built, record sheets, laboratory reports
and other inspection related certificates during performing of the job to the
Engineer for his approval. Engineer is responsible to advise GIS Department
any changes to these approved As-built and record sheets.

H) Handing-over of Site
All sites proposed in this tender shall be considered as handed-over to the Contractor
on the date of award. It is therefore, the responsibility of the Contractor to depute his
Surveyor/Engineer to receive the locations/sites as early as possible upon receiving
WO's from the Engineer. The Engineer shall determine priority of each site for
execution during the contract.

I) Pre-tender Clarifications
Before submitting the tender, Tenderer must make independent inquiries as to the
scope of works by visiting the site, the prevailing local conditions and generally obtain
their own information on all matters that may in any way affect their pricing, completion
period, risks and obligations and assess themselves for the extent of work involved.
The tender price shall be deemed to include all that is necessary to supply and carry
out the works to an acceptable level and in a safe manner observing all associated
government regulations. No claim will be considered on the ground due to lack of
information.

J) Contractor's Personnel
All Contractor personnel must have appropriate trade qualification or University
degrees in the applicable disciplines and must have sufficient years of experience in
the assigned scope, as per following requirements but not limited to;
All the contractors staff qualification are subjected for Engineer review and approval.

1) Project Co-ordinator
QUALIFICATIONS/SKILL
The suitable applicant shall be qualified with a B.Sc. Civil Engineering Degree
from a recognized University. Background of construction project controlling and
leadership capability.

EXPERIENCE
He should have had experiences in field of water and/or wastewater with relevant
experience in Asset Tagging.

K) Other Employer's Requirements

1) Media Documentary (Photographic & Video)

The Contractor shall obtain a photographic and video record showing the
progress of work every week and month, and / or as necessary, in order to
provide sufficient visual evidence of the progress and to record the specialized
events features of the works or other matters of interest in connection with the
works or their surroundings of the project. Such media documentation shall be
submitted to the Engineer in the form of an accepted media documental
presentation, as and when required during the progress of works and as a
complete package at the completion of the project.

2) Notice to Public on Road Closures

Upon approval from Engineer's representative on any road closure for the
purpose of carrying out the Works of PWA, the Contractor shall place a
sufficient notification to the general public in local newspapers both in Arabic

and English languages at least one (1) week prior to the road closure. The
contents of notification shall be approved by the Engineer and the extent shall

not be less than a quarter of the standard size page of a reputed local
newspaper. In order to evident the notification the Contractor shall submit to the
Engineer a copy as appeared on the newspaper on the same day of the
notification.

3) As-Built Record Drawings (QCS SECTION 1 Part 7)

The Contractor shall prepare 'As-built' photos, documented evidence of the


Works detailing all works installed required under the Contract including
services and all level.
As-built record drawings, photos and the operation and maintenance including
manuals and any other literature required for the efficient operation of the
completed works shall be supplied by the Contractor to the Engineer free of
cost. The documents shall be durable and easily reproducible medium, and a
computer disk with all relevant data in a format approved by the Engineer.

One additional copy of all project documents, drawings and reports are to be
submitted in an approved format recorded on computer media (Compact Disks)
using software which is compatible with the Document Control Unit computer
system.
The Contract shall not be considered as complete until the Contractor submits
to the Engineer the as-built drawings and documentation, and obtains the
Engineer's approval.

Four copies of as-built drawings and documentation shall be submitted to the


Engineer before the expiry of the Contract Period as named in the Contract and
shall include one copy of each drawing and document specifically marked for
archival storage. The Certificate of Maintenance (FCC - Final Completion
Certificate) will not be signed by the Engineer until all as-built drawings and
accompanying documents and service literature are approved and a certificate
has been issued by the Engineer to this effect.

Submittals shall meet the requirements of BOQ items 2.1, 2.2 and of this
Section.

7) Project Sign Board (QCS SECTION 1 – Part 12.3)

The name of the Employer shall be Public Works Authority and the name of the
Engineer shall be Director of Assets Management Affairs.

8) Construction Cleaning (QCS SECTION 1 – Part 15.2.1)

Further to the requirements of SECTION 1 – Part 15.2.1 of Qatar National


Construction Specification and Conditions of Contract Clause 33 (3), the
Contractor shall clear the site and any buildings thereon of all materials, plant,
rubbish and building debris of any nature to a tip designated by the relevant
Municipality for the area and shall ensure the area surrounding the Works is
clean and to the natural level of the surrounding land.
If the Contractor fails to comply with the foregoing requirement within 30 days of
Practical Completion of the Works the Engineer may, in addition to the terms of
the Conditions of Contract Clause 33(3):
(a) Clear the site using another Contractor and deduct the costs so incurred
from any monies due to the Contractor, and
(b) Delay final payment to the Contractor until such time as the Site has been
cleared to the satisfaction of the Engineer and relevant Municipality.

9) OTHER GENERAL SPECIFICATIONS AND REQUIREMENTS

Specifications
The Tenderer is to note that all the specifications stated in the Tender
Document are to be considered the minimum specifications and requirements
required.
All work to be executed under this Contract shall be carried out in accordance
with the QCS amplified or amended by the Project Specifications of the Tender
Document.

General Requirements

Protection of Existing Services


Contractor shall exercise necessary precautionary measures to protect the
existing services and in an event of any damage, the contractor shall bear the
consequences and reinstate the same on his own expenses to the satisfaction of
the relevant authority.

Safety
Safety at the site is sole responsibility of the contractor. Work should comply with
QCS, Roads, Traffic police, Municipality, Drainage Affairs Standards & all

including for work along roads at in storm water lines and pumping stations.

Method statement

After award of the Contract, the Contractor shall submit a method statement
incorporating the risk assessment and plan for the work for the Engineer’s
approval.

Materials

All materials shall be from approved suppliers as per Assets Management Affairs
approved list or of the best quality.

Laboratory Testing

Only an approved Laboratory by the Assets Management Affairs shall be


employed for any testing as above.
Over Pumping

Contractor has to arrange all equipment, tools, and materials required to


complete the work in proper manner including over pumping arrangement

1.0 GENERAL

1.1 Responsibility
1. It is the Contractors responsibility to conduct his operations in such a manner so
as to avoid danger or injury to the public and personnel employed at the work or
any other persons or even damage to property. The Contractor shall take
precautionary measures for protection against such risks and shall inspect
safety conditions where the works are being executed.
2. Contractor shall insure the safety of pedestrians, vehicle, traffic and properties
near the working area and especially if there work inside live sewers or any
excavation, pipe laying or aboveground construction as part of this Contract.

3. All Contractor’s personnel must be able to work safely at all times within the
strict environment of sour gas and their key/Senior staff shall be certified in the
use of breathing apparatus / Gas Detectors and precaution action on confined

spaces restriction and must hold current certification issued by O&M

4. Throughout the contract duration and any extensions thereof, Contractor shall
keep the sites clean and free from all unnecessary obstruction and shall store or
dispose of contractors’ plant equipment and hand-tools in a safe and orderly
manner.

5. The Contractor shall conform to all Acts, Orders and Regulations made by any
official authority of Qatar with respect to health and safety.

1.2 Safety Officer


1. Safety officer is subjected to approval by Engineer (required qualification and
experiences is marked on "Staff Qualification" part.

1.3 Reporting Accidents


1. The Contractor shall promptly report to the Engineer any accident whatsoever
arising out of, or in connection with, the Works whether on or adjacent to the
Site which caused death, personal injury or property damage, giving full details
and enclosing statements of witnesses.

2. Promptly shall mean immediately where it impacts on the operation of a live


pumping station or treatment plant and in all cases never more than 24 hours.

1.4 General Sanitation


1. The Contractor shall maintain the Site and all working areas in a sanitary
condition and in all matters of health and sanitation shall comply with the
requirements of the Ministry of Health and the Drainage Affairs.
2. The Contractor is responsible for all measures necessary to prevent the
breeding of mosquitoes on the Site for the duration of the Contract.

1.5 Safety Notice Board


1. The Contractor shall set up and maintain throughout the course of the Contract
safety notice boards in prominent places on the Site. These notice boards shall
be located in positions approved by the Engineer such that they are clearly
visible to the Contractor’s employees.
They shall be fully illustrated and provide details of key safety procedures to be
followed.
2. The notice boards shall be in Arabic and English.
3. The Contractor’s employees shall be made fully aware of the notice boards prior
to commencing duties on Site.

1.6 Compensation for Damage


1. Claims for compensation arising from damage or injury caused by the
Contractor’s failure to provide adequate health and safety measures shall be the
sole responsibility of the Contractor.

2. The Contractor shall notify the Engineer of any claim made against him which is
in any way connected to the Contract.

2.0 SAFETY PLAN

2.1 Safety Plan

1. The Contractor shall submit a Safety Plan to the Engineer for approval within
ten (10) Days of the award of the Contract.
2. The Safety Plan shall include the Contractor’s proposals for the maintenance of
safety on the Site. These proposals shall address the safety measures
applicable with respect to all tasks to be undertaken.
3. The Safety Plan shall include the Contractor’s proposals for accident
prevention. Accident prevention shall include but not be limited to training,
monitoring and review of safety related procedures, enforcement of safety
related matters and promoting safety awareness.

2.2 SPECIAL SAFETY PRECAUTIONS


Permits

1 A permit-to-work system shall be obtained when undertaking work at start of


project to deal with existing utility, service, item of equipment or structure and
work in a confined space.
2 The Contractor shall provide all personnel with identification cards bearing the
employees photograph and contract number to the approval of the Drainage
Department.

2.3 FIRST AID


2.3.1 First Aid Stations
1 The Contractor shall provide and maintain first aid stations complete with all first
aid equipment necessary for the initial care of any of the Contractor’s or
Engineer’s personnel who may be injured.
2 As a minimum, there should be a first aid station in all areas where work is in
progress.
3 The Contractor shall ensure that the first aid equipment at each first aid station
is complete in all respects at all times.
4 At each first aid station the names of the Contractor’s staff that are trained to
render first aid shall be displayed.

2.4 FIRE PRECAUTION AND PREVENTION

2.4.1 General
1 The Contractor shall take all necessary precautions against fire as required by
the insurance company insuring the Works, the Department of Civil Defence
and the Engineer.

2 Quantities of flammable materials on site shall be kept to an absolute minimum


and shall be properly handled and stored. Any handling and storage
recommendations made by the manufacturers of flammable materials shall be
strictly adhered to.

3 Except as otherwise provided herein, the Contractor shall not permit fires to be
built or open type heating devices to be used in any part of the Site.

4 The Contractor shall maintain fire fighting equipment in the site offices, the
stores and about the Works where applicable. The type, amount and location of
fire fighting equipment shall be to the requirements of the Department of Civil
Defense. The Contractor shall confirm to the Engineer in writing that the
requirements of the Department of Civil Defence have been met, enclosing the
approval letter from Civil Defence Dept.

5 The Contractor shall make arrangements with the Department of Civil Defence
to inspect the Works and promptly carry out their recommendations at his own
expense if requested to do so by the Engineer.

2.5 SAFETY OF THE PUBLIC

2.5.1 Working in Public Areas


Safety and security procedures shall be implemented as required by the
Department of Civil Defence and the police. These shall be included, as a
minimum, warning signs and lights, barricades, railings and other safeguards as
required by the nature and location of the work being undertaken.

2.6 Safety Equipments

2.6.1 Safety Equipment, Protective Attire and Personnel Protective Equipment


(PPE):
1. The Contractor shall provide all personnel with suitable safety clothing including
but not limited to overalls, hard hats and safety shoes. These must be worn at
all times while on site.
2. The contractor has to designate one person (area in-charge) who is fully trained
in the use of all safety equipments, tools and materials and he should hold a
valid certificate in the use of Breathing Apparatus / Gas Detectors and
precaution action on confined spaces restriction and must hold current
certification issued by O&M Department Safety Engineer or completion of
relevant courses.
3. The Contractor shall provide all safety equipment in accordance with Drainage
Department safety regulations, to carry out daily routine preventive maintenance,
all other related operations any works in Confined Spaces which is to include but
not be limited to the following:
(a) Safety helmets, safety harness and ropes, rubber gloves.
(b) Portable ventilation equipment.

(c) Gas Detectors/monitors complete with carrying case, rechargeable


batteries and battery charger, calibration kit, all to the approval of the
Engineer; consumable items and sensors to be replaced in accordance with
the manufacturer’s recommendations (Gas detectors for all the working
groups.)
(d) First aid kits, for the groups and operators
(e) Safety and hazard notices for the groups and operators
(f) Full safety harness with lifelines and shackles
(g) Lifting frame complete with fall arrest device
(h) Constant flow escape Breathing Apparatus complete with storage cases
and Gas Masks for the Sewer work
(i) Automatic positive pressure self contained breathing apparatus complete
with storage case
(j) Torches and Signal lights
5. The Contractor will be responsible for the maintenance, calibration, etc of all
safety equipment tools and materials, etc.
6. Correctly made ladders must be provided for access into tanks, onto scaffolding
and buildings. Contractor’s self-made ladders shall not be used.

7. Scaffolding shall be erected in accordance with the manufacturer’s


recommendations and shall be fitted with toe boards and hand railing.

8. Safe operational of crane facilities (SWL) to be certified regularly by Authorized


TPA.

2.6.2 Support Facilities for Contractors Staff and Labor


1 The Contractor shall provide and maintain for the duration of the Contract
sufficient sanitary facilities for use by workmen in accommodation areas, at
messing facilities and in areas where work is in progress.
2 The Contractor shall provide and maintain for the duration of the Contract
messing facilities and a separate designated area for the consumption of food
for his staff and laborers.
3 The out site accommodation of contractor's staff and/or when allowed that, the
Contractor may provide accommodation facilities for his staff and laborers at the
Site, such facilities shall be maintained in a proper manner and to the
satisfaction of the appropriate government departments and the Engineer.
4 The Contractor shall provide transportation between accommodation and areas
of work for his staff and laborers.
5. The contractor shall provide two pieces of overalls, one pair safety shoes to the
staff every year.
6. The contractor shall provide three nos. of cotton gloves to the staff every month
and also washing soap. Dettol for personal hygiene.
7. However contractor is responsible to provide all required PPE and other essentials
to the workers, as safety is first.

3.0 MOBILE COMMUNICATIONS

The Contractor shall provide GSM (mobile telephone) to all Engineers and Key
Staff.
