ACKNOWLEDGEMENT OF DEBT
AND PROMISSORY NOTE

I, CORNELIA B. GARCIA, of legal age, married, Filipino citizen, and

resident of ______________, after having been duly sworn to in
accordance with law, do hereby depose and state, THAT:

1. I am public school teacher currently assigned at Masalasa

Elementary School, Masalasa, Victoria, Tarlac;

2. Sometime on ____________, I requested my co-teacher, namely:

Jennilyn D. Rigor, Myra G. Gagarin and Dalen E. Tolentino to serve
as co-makers in my loan application with Manila Teachers Mutual Aid
System (MATMAS), to which they agreed;

3. Subsequently, I prepared the corresponding MATMAS loan

application form and had it signed by the above-named co-makers;

4. However, without the above-named co-makers noticing it, I was not

the applicant indicated in the loan application form they signed but it
was Editha Mauricio;

5. I had the loan application form of Editha Mauricio signed by the

above-named co-makers, because she (Editha Mauricio) at that time
needed a considerable amount of money to attend to an emergency
situation;

6. However, Editha Mauricio became AWOL and thus payment of the

loan obligation was charged to and collected from the above-named
co-makers through salary deduction;

7. Acknowledge that above-named co-owners are unknowingly victims, I

hereby undertake to pay the amount being automatically deducted
from their respective payrolls under the following terms:

8. Should I default in the payment of my obligation to the above-named

creditor, the MATMAS is authorized to charge the corresponding
deduction on my payroll account.
