AGREEMENT FOR THE INSTALLATION

OF SERVICE ENTRANCE UNDER AMC PROGRAM

THIS Agreement is made and entered into in , Philippines, this day of

REGEN ENGINEERING SERVICES, a duly organized under and by virtue

of the laws of the Philippines and accredited by Meralco to install service entrance under
the Accredited Meralco Contractors (AMC) Program, with office at

referred to as the CUSTOMER.

The parties have contracted and agreed as follows:

1. AMC shall install the service entrance of the CUSTOMER at the premises of the CUSTOMER located at

2. AMC shall facilitate the processing of application for electric service connection by securing all the necessary
documents, including right-of-way grant, if necessary, and submitting the same to MERALCO in behalf of
the CUSTOMER.

Furthermore, the installation of a service entrance must be in compliance with the standards and specifications
of Manila Electric Company (Meralco) and in accorda nce with the provisions of the Philippine Electrical Code.
4. The CUSTOMER shall pay the AMC the sum of (in
Pesos) representing the total contract price for the aforementioned work.
5. Should the CUSTOMER be in default in the payment of the contract price, AMC may retire the service entrance and
forfeit any advance payment.
6. This agreement shall also be subject to the Terms and Conditions of Service Entrance Installation of AMC
contained at the back and made part of this agreement.
7. This Agreement shall be in full force and effect from and after the CUSTOMER paid the required down payment of
the contract price.
