Pre-Test:

The pre-assessment for this unit was an emoji pre-assessment. The teacher went over what each
face meant before allowing students to circle how they felt about each science vocabulary word.
The students were then to pick 1 out of 5 emoji faces on solid, liquid, and gas. A lot of students
did not have smiley faces. Mostly all students, circled a confusion face or sad face which means
they really had no idea what each word meant. Only two students circled a smiley face, which
means they do know the vocabulary word.

Lesson One Formative Assessment:
The formative assessment for the first science lesson is an exit slip. The students will compete an
exit slip over the jeopardy game they played with their groups, which they had to pick different
topics and answer questions depending on that topic ( https://jeopardylabs.com/play/3rd-grade-
matter-26 ) After students complete the exit slip, the teacher will go over what students have
wrote, to address in the second lesson.

Lesson Two Formative Assessment:


The formative assessment for the second science lesson is a Kahoot quiz. The students are
assessed through an online website, which allows for the teacher to know what students are on
task and what students are struggling. They will be given each the Kahoot quiz on their own
computers. Through the computers, the students will answer 6 questions that apply to the
property sort they did previous to the Kahoot quiz. This will allow students to apply what they
just learned to an online quiz.

Lesson Three Formative Assessment:


The formative assessment for the third science lesson is a “ticket out the door”. The students
participated in an experiment and made Grinch slime. Through making the slime, the students
witnessed and discussed different states of matter. On students ticket out the door, they had to
discuss one state of matter on their ticket they saw in the Grinch slime and what happened
throughout the process of making that state of matter. (ex: did it get hard? Or soft?)

Lesson four Summative Assessment:


The summative Assessment for the fourth science lessons is students (pre-cut words and
sentences in an envelope) pasting and matching the correct word to the correct sentence on the
guided worksheet. The students are assessed at how many they correctly match together. Rubric
is shown below.
