ABSTRACT

The Effect of Leg Elevation on Preventing the Decrease of Blood Pressure in


Spinal Anesthesia Patients in the Surgery Room of Penajam Paser Utara Public
Hospital

Background: Spinal Anesthesia is an anesthesia treatment given a surgery. One of the


complications which usually occurs is the instability of blood pressure, namely the
decrease of blood pressure to the hypotension degree. One of the ways to prevent the
hypotension is giving a non-pharmacological treatment in the form of leg elevation.
Objective: To find out the effect of leg elevation on preventing the decrease of blood
pressure in spinal anesthesia patients in the surgery room of Penajam Paser Utara
Public Hospital.
Method: This research used quasi experimental method with a two group pretest –
posttest design. The sample of this research was all perioperative patients who got spinal
anesthesia, taken by using consecutive sampling method with the total of 24 patients.
The data were collected by using observation and were analyzed by using independent
t-test.
Findings: The research findings revealed that the systolic blood pressure of the patients
who did leg elevation at minute 5 was 97 mmHg; 101.35 mmHg at minute 10, and
106.98 mmHg at minute 15. The diastolic blood pressure at minute 5 was 57.25 mmHg,
59.50 mmHg at minute 10, and 64.83 mmHg at minute 15. The systolic blood pressure of
the patients who took medicine at minute 5 was 101,46 mmHg; 95.88 mmHg at minute
10, and 104.96 mmHg at minute 15. The diastolic blood pressure at minute 5 was 58.29
mmHg, 56.04 mmHg at minute 10, and 61.21 mmHg at minute 15. There was no any
difference of blood pressure between patients in the experimental group and those in the
control group in the systolic blood pressure at minute 5 (p value = 0.251), at minute 10 (p
value 0.121), at minute 15 (p value = 0.574). There was no any difference of diastolic
blood pressure at minute 5 (p value = 0.574), at minute 10 (p value 0.612), and minute
15 (0.068). However, there was an effect of leg elevation on the increase of blood
pressure in the experimental group from minute 5 to minute 10 (p value 0.001) and from
minute 10 to minute 15 (p value = 0.001). There was an increase of diastolic blood
pressure from minute 5 to minute 10 (p value = 0.018) and from minute 10 to minute 15
(p value = 0.000).
Conclusion: There an effect of leg elevation on preventing the decrease of blood
pressure of spinal anesthesia patients. It is recommended to the Penajam Paser Utara
Public Hospital to apply leg elevation as an alternative of non pharmacological treatment
to the spinal anesthesia patients.
