I am the parent/guardian of the child named above, and give permission for her/him
to participate in the The BEAST 5K RUN on December 21.
Parent Consent: I hereby give permission for my child named above to participate in
the Fun Run. In consideration of my child being permitted to participate, I agree to
hold harmless from any liability the Supreme Student Government, and Camiling
Colleges, Inc., faculty and staff, attest and verify that my child is physically fit to
participate in the Fun Run.
