General Discussions:

1. Submission of action plan for Ground Floor Casting by ALML is pending. ALML argued about
the validity of the action plan if can’t be executed due the fact that ADC is always obstructing
the work progress. In response Md. Bahauddin, Project Manager from ADC cited that ADC
only is responsible for checking quality of work as well as certain approvals but ALML is
responsible for the construction time. If ALML prepares the activity in hand as per norm code
then ADC certainly will not stop the work. UNICEF also added that in their last site visit they
found that ADC checked the thickness of retaining wall and it was found to be 9” to 9.5”
instead of 10” in some portion in this case ADC has no option but to issue NCR or provide
onsite rectification plan based on which ALML has to resolve the issue. For this issue if the
casting is delayed then ADC is not responsible.

2. As with the present batching plant, ALML is facing problem to finish casting the targeted
segments and most of the time another construction joint is needed due to that issue. As
such ALML is required to have additional construction joint formwork for construction work so
that it can be installed in mat when casting is stopped due the issue of batching plant or time
for casting exceeds the approved time period.

3. Active participation of Sub-Contractor is required. It has been observed many times that
during casting or any activity related to casting, Sub-Contractor is not present at site or not
instructing his workers properly. ALML should ensure that, active participation of Sub-
Contractor.

4. Casting of mat should not be started early in the morning as the construction joint formwork
is to be removed and be prepared with bonding agent before casting of next segment starts.

5. ALML is solely responsible for the quality and delay of the work whether it is their
shortcoming or their appointed Sub-Contractors’.

6. Removal of spill out concrete and loose concrete over the unfinished segment of casting
(when casting cannot be completed within approved time period) is a must.

7. As per previous decision, revised mat casting completion date is 10th November, 2018.

8. Batching plant quality regarding operation is not satisfactory and also it appears to be an old
one.

9. ALML have to work out a temporary solution to support the casting of mat. But along with
that they need to take a permanent solution of this batching plant issue.

10. More construction joints are appearing than the proposed action plan submitted by ALML.

11. Mixture machine with hopper is a temporary solution and is not permitted for concrete
casting of Superstructure. ALML was provided with a deadline of 04.10.2018 to solve the issue
of batching plant but still no significant action from ALML.

12. Manual concreting of column and retaining wall is approved as it is possible to ensure quality
of the casting better than the present batching plan.

13. ALML has to submit hard copy of the 06 (six) test reports received on 4th November, 2018 to
ADC.

14. ALML cannot outsource ready mix concrete. All concrete is to be prepared and poured on site.

15. ALML will submit their methodology for casting the Ground Floor Slab especially the folded
portion of the slab which has a level difference of 2’-6”.

16. No construction joint in slab is permitted.

17. ALML must use newly fabricated shutters and remove old shutters from site, if any. But they
may keep very few shutters for staging purpose but those have to be marked with separate
color so that it can be easily identified.

18. ALML was requested to provide a QC Engineer by both UNICEF & ADC within 16th October,
2018 but they still did not appoint one. ALML mentioned in the meeting that, Mr. A.K.M Sohel
Ahmed, Manger Construction, is also fulfilling the role of QC Engineer. In response UNICEF
added that if he is fulfilling the role of a QC Engineer then why the thickness of retaining wall
was deviated from the thickness specified in the Structural Drawings.

19. Mr. A.K.M Sohel Ahmed, Manger Construction, mentioned that he has already drawn
attention to their top management to appoint a QC Engineer.

20. If construction work is stopped either due the quality issue or any matter identified as
detrimental to the structure UNICEF is to be notified as soon as possible.

21. QA/QC plan, Methodology etc. submitted during the tendering process (basis of which ALML
is selected) has not come to fruition till now. There is a huge difference in what they have
proposed and what they are doing. ALML must abide by their proposed plan, methodology
etc.

22. UNICEF suggested ALML to provide samples of the fixtures of the brands recommended by
ADC. Also ALML should mention the Code number for the sample.

23. UNICEF suggested ADC to mention why a certain brand or sample is recommended or not
recommended.

24. Specification of water stopper was provided in time and also the sample submitted by ALML is
approved by ADC. .

25. ALML must submit specification of water sealant for approval.

26. ALML should provide methodology and action plan for casting blast wall on west side without
removing the temporary fence.

27. Construction of blast wall on the south side will require removal of the existing boundary wall.
ALML need to provide methodology and work plan for approval.
