    TUITION WAIVER AFFIDAVIT

    By signing this affidavit I understand:


    1. That I currently possess a baccalaureate/master's
    level degree.

    2. That by utilizing the ACT/URI-NEA tuition waiver,


    those courses in which I may enroll will not be
    utilized to obtain an additional baccalaureate/
    master's level degree.
