JOINT AFFIDAVIT

That we, GERLITA CAL and RAYMUND GERALDEZ, both of legal age,
Filipinos, married and single, respectively, and residents of Sitio Cangipos, Barangay
Balugo, Sibulan, Negros Oriental, under oath, do hereby depose and manifest:

That we personally know JULITO SERINIO CIANO who was born on August 2,
1971 to the late spouses Marcelo Ciano and Felisa Serinio in Sitio Tinapayan, Balugo,
Sibulan, Negros Oriental;

That he is known in our community as JULITO SERINIO CIANO, which is his

true and correct name and he used in all his official transactions and school records;

That his first name was erroneously entered in his Live Birth Certificate as
JULIO, instead of his true and correct first name JULITO and the latter first name was
the one he used in his marriage contract with Myrna E. Gomez;

That the family name of his mother SERINIO was also erroneously entered in his
Live Birth Certificate as SERENIO, instead of SERINIO which is his mother’s true and
correct family name;

That the name JULIO SERINIO CIANO appearing in his Live Birth Certificate
and the name JULITO SERINIO CIANO, appearing in his Marriage Contract, refers to
just one and the same person; likewise the family name of his mother SERENIO
appearing in his Live Birth Certificate and the Family Name of his mother SERINIO
appearing in his Marriage Contract, refers to just one and the same person;

That we know all these facts because we are good friends and neighbors in Sitio
Cang-ipos, barangay Balugo, Municipality of Sibulan, for quite sometime already and
until the present;

That we executed this affidavit freely and voluntarily without any monetary
considerations or promise whatsoever, nor executed to defraud any agency of the
government of the Republic of the Philippines; but to attest the truthfulness of our
statements herein.
