DIALOG FOR MIDTERM
Identifying a patient’s chronic problem
Date: May, 6th, 2009
Patient: Good morning

Doctor: Good Morning Ms. Stefany, Please sit…

Doctor: How are you today?

Patient: I am so worried…

Doctor: Why? Tell me about your medical problem…

Patient: I have the same problem, I m taking a lot of medication, but the acidity is always
there.

Doctor: I see, all your visits and problems are registered in your medical history. Now, I
believe your medical problem is ‘chronic’.

Patient: What does ‘chronic’ mean, doctor?

Doctor: It means going on and on over a period of time, often quite a long period of time.
A ‘chronic’ condition is one that is of long standing.
It is the opposite of ‘acute’, which we use to mean something that has arisen suddenly –
out of the blue, you might say.

Patient: I see. Well, mine’s definitely chronic then.

Doctor: Yes, I should say so, by now. So – does this acid manifest itself straight after any
meal in particular? Say, after breakfast?

Patient: Yes, I suppose it does. I remember telling you before – it is always at its worse
straight after breakfast.

Patient: And it’s just the same now. But I can get it any time

Doctor: Yes I remember you telling me. So - mainly after breakfast. Now, have you
noticed if any particular foods trigger the heartburn especially fats? Or liquids?.

Patient: Well, coffee and tea don’t seem to have much effect on it. If I drink a little bit to
much the night before, that can sometimes bring it on bad the morning after.

Doctor: What do you normally drink?


Patient: Oh, beer, beer – of course. But never more than a pint or two. Sometimes a glass
or two of wine.

Doctor: The first step is to leave the beer and wine. Your living style is not the best way
to feel better, it is reason of your problem.

Patient: I know this, doctor, I will try to quit.

Doctor: Now, I recommend that you take all your medications and add this in your daily
medication, before you eat.

Patient: Thanks, doctor.

Doctor: I will see you in 2 weeks and tell me about your problem. Good Luck and have a
nice day.

Patient: Bye, doctor.
