Preconditions: The Physician Family Clinic is transitioning from a paper-based patient health record
system and paper-based billing, laboratory records management system

Basic process flow for patient check in:

1. Clinical staff/nurses and doctors migrate paper-based records to Electronic Health Records (EHR)
2. Clinical staff uses EHR to verify current or new patient
3. Clinical staff/nurses review patient’s insurance, demographics and arrival time
4. Doctor reviews EHR – prior diagnosis, lab results, treatment options
5. Doctor and clinical staff/nurse review EHR record, doctor takes notes, nurse assigns room
6. Patient arrives, takes EHR IPAD and checks-in, pays co-pay, waits for doctor

Alternative flows
1a. Physician Family Clinic hires outside staff to migrate paper-based, old records to EHR
2a. Patient is new and clinical staff/nurses provide EHR IPAD to new patient in order to complete intake
forms, provide demographics, insurance information, prior treatment, prior doctor, etc.
3a. Patient is new, without insurance or does not have insurance information available
4a. Patient is new and the clinic staff/doctor set up EHR record for examination and preliminary diagnosis
5a. Doctor takes hand-notes and enters the notes after reviewing the patient’s diagnosis and develops an
online treatment plan
6a. Patient arrives, informs clinical staff/nurses of current medications, clinical staff/nurses consult with
doctor, doctor adds medications on EHR, writes prescription and patient leaves
