1. INTRODUCTION

This guideline describes the communication channel to be used for inspection and maintenance
during shutdown and rotation of inspection and maintenance work books which should be prepared
in accordance with procedure MOG-HSEQ-Q-P-284 “Corporate Procedure for Technical
Inspection”. All planned activities during shutdown” shall be completed in accordance with work
book and inspected and certified by certifying body. This applies to all plant and equipment that are
considered to be „safety critical‟ or where it is mandatory to certify the equipment by the statutory
authority at regular interval in accordance with Company guideline MOG-HSEQ-Q-D-243
“Corporate Procedure for Plant Inspection Philosophy”.

2. OBJECTIVE
Scope of this guideline is to identify responsibilities of concerned departments and to establish
communication channel within the departments so that the planned activities are completed in
accordance with relevant procedures within the shortest period.

3. AREA OF APPLICATION
This procedure is applicable to all activities and processes of the Mellitah Oil and Gas BV‟s (MOG)
related to Communication for Inspection and Maintenance activities during Shutdown both external
and within the Organization including contractors and subcontractors that work within MOG sites.

Following is a list of abbreviations commonly used in this procedure, and is designed to help the
reader with their understanding of this document.

vessel / equipment.

6. RESPONSIBILITIES

The responsibilities relevant to the present procedure are outlined in the following paragraphs:

Complex/Field Manager
Complex/Field Manager is responsible to ensure all departments within the Complex/Field are
aware of the objective of inspection and maintenance work book and implemented are in
accordance with Company procedures.

HSEQ Division Manager/ HSEQ Department Managers (Oil and Gas)

HSEQ Division Manager/ HSEQ Department managers (Oil and Gas), each in his area, or his
delegate has the responsibility to review the work books. He is also responsible to ensure
availability of certifying body at site during shutdown activities for certification of all SCE
components in accordance with Company Procedures.

Maintenance Manager
Maintenance Manager or his delegates at site is responsible to ensure all planned maintenance
activities are performed in accordance with maintenance work book and SCE are made available
for inspection.

Engineering Manager
Engineering Manager or his delegates is responsible to establish the inspection program, prepare
work book and perform inspection activities as outlined in the ITP/Procedures. He should collect
inspection data and summarize the deterioration history. He shall maintain all records of inspection
and test. He shall coordinate with HSEQ Dept manager for all quality issues. He is also responsible
to review inspection results and recommend further disposal such as use as is, replace, modify,
repair, redundant etc,

General Maintenance Service Contractor (GMS)


Maintenance Contractor is responsible to provide assistance, lighting, cleaning, scaffolding and
other facilities required for safe entry and inspection. Maintenance Contractor shall prepare Method
Statement and Work Instruction for all inspection activities and submit to Company for approval.
Contractor shall strictly follow the work books for maintenance and inspection activities.

Certifying Body
Certifying Body is responsible to review the inspection work book and approve prior to any major
repair work is performed. CB shall ensure the proposed repair and inspection procedures are in
accordance with Code or statutory requirements. CB may inspect the vessel or equipment during
and/or after the repair work / cleaning process and confirm that the repair work has been completed
in accordance with approved procedures and/or code. CB is responsible for certification/re-
certification of vessel/equipment.

7. GENERAL GUIDELINES AND FLOW CHART

 Engineering Department shall prepare the inspection work book in accordance with
requirement of Company procedure MOG-HSEQ-Q-P-284
 HSEQ shall review the inspection work book and forward to the Certifying Body for final
approval. Any comments or additional requirements from CB shall be incorporated and sent
to Engineering Department for implementation.
 Maintenance Department shall prepare maintenance work book as per Company Procedure
MOG-HSEQ-Q-P-284 and forward to HSEQ for review.
 Maintenance to start planned activities first. After opening the vessel, maintenance will
inform Engineering for inspection.

 When a vessel or equipment is inspected by Engineering, a preliminary report shall be


prepared and included in the inspection work book. In case of any repair such as welding is
involved, Engineering shall prepare all relevant repair and welding procedures. The
inspection work book shall be forwarded to HSEQ for review.
 At the discretion of CB, the CB may enter the vessel any time during the repair activities. All
Company HSE procedures must be followed for Vessel Entry.
 Engineering shall inspect the vessel after cleaning or repair and prepare a final inspection
report. Company Plant Inspectors should witness intermediate stages of inspection/test as
required.
 Maintenance shall ensure all attachments are re-instated and the vessel is ready for box-up.
 CB shall review the reports and certify. Incase of any anomalies or conditional certification,
the same shall be reported to HSEQ-Tripoli.
 Engineering shall inspect the completion and release the Vessel/Equipment for box-up in
coordination with HSEQ.

8. COMMUNICATION

Since shutdown is a time bound activity, all parties should coordinate with each others in
accordance with flow chart and expedite their work. Written confirmation by emails between stages
given above is not required that may impact on the work schedule.

9. RECORDS

Engineering Department shall maintain all inspection work books and prepare a data base giving
complete inspection and certification details. Maintenance Department shall maintain all
maintenance work books and prepare a data base of the maintenance activities performed during
shutdown.
