To whom it may concern,

I, Ms. Jules Ann Felice T. Chua who works as a Speech and Language Therapist at Speciabilities
Development Center Incorporated located at _______, with telephone number _____, sister of Mr.
Jehrard Fillipp Tiu Chua who is a Filipino citizen holding Philippine passport no. ________ issued in
DFA NCR West, Philippines, and presently residing at, ________, after having been duly sworn to in
accordance with the law, do hereby depose and say:

That I wish to support the following person during our trip to South Korea:

Mr. Jehrard Fillipp Tiu Chua

That I know personally the above-named person who is my brother;

That I hold myself responsible for his financial expenses, accommodation, and support during his
stay in Korea;

That I guarantee that he will abide by all law and regulations by the Korean government;

That I further guarantee that I will pay for his airfare back to the Philippines and other related
expenses;

That this affidavit is being executed to attest to the foregoing facts and not for the purpose of
FRAUD but in connection with the visa applications of the above-named person.

Attached is a photocopy of my certificate of employment, and work ID; as well as an original copy of
my bank statement.
