Copy of certified Marriage Certificate showing date of marriage

Legal spouse AND

Copy of tax return showing filing status

Copy of Notarized Domestic Partnership Affidavit

AND

Copy of one of the following documents showing financial interdependence

• Documentation of current joint ownership of residence
• Documentation of joint tenants on current lease of residence
• Copy of both driver’s licenses reflecting the same address
Domestic Partner
• Current bank/credit card statement (within the past 12 months) with both names
• Current utility bill (within the past 12 months) showing the same address as the
• Current automobile title or registration showing joint ownership
• Mortgage statement or property rental agreement with both names (within the past 12
months)
• Current loan note or payment coupon demonstrating joint obligation of the loan
(within the past 12 months)

Child Under 26 years

Copy of certified Birth Certificate naming you as parent.

Official Hospital Birth documentation acceptable if it names you as a parent, includes

child name and date of birth (acceptable only for children under 6 months of age).

Copy of certified Birth Certificate naming your spouse as parent and a copy of your

Domestic Partner’s Copy of certified Birth certificate naming your Domestic Partner as parent and a copy
