SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

I, AAAA , of legal age, Filipino, and residing at _______________________do


hereby NAME, APPOINT, and CONSTITUTE the Law Firm of______________, its
attorneys, associates and/or representatives, with office and business address at 4/F
____________________________________to be my true and lawful attorney-in-fact and
in my name, place and stead and for my own benefit, to do and perform the following
acts and deeds, to wit:

1. To represent me in the case _________________docketed as Civil Case No. R-


____________________, pending before the Regional Trial Court of Bulacan
Branch 99, of which I am the petitioner;

2. To execute and sign verification and certification of non-forum shopping of


pleadings subsequent to the petition as may be required by the rules of
procedure in relation to the above case and to verify and certify any and all other
pleadings;

3. To litigate any cause of action which may arise in relation to the above- entitled
case or to any incident involved in the same with the trial courts of competent
jurisdiction, the Office of the City Prosecutor, or any government agency,
including all the proceedings thereof, as well as in any appeal, petition for
review, or special civil action arising therefrom or as a consequence thereof, and
to secure the aid, assistance, or services of a lawyer, or lawyers to represent me in
the said and all related proceedings;

4. To file, prosecute, defend, intervene in any and all other cases, civil or otherwise,
related and/or connected with my defense or to any of the incident involved in
the above-entitled case, and to verify and certify pleadings in connection with
any petition, complaint, answer, reply, rejoinder, or any similar documents in
relation to the filing of complaint, preparation of answer as defense or
intervention involving the above-entitled case or any of the matter involved in
the above- entitled case;

5. To appear for and in my behalf in the preliminary conference, pre-trial,


mediation proceedings, and Judicial Dispute Resolution (JDR)of the above-
mentioned case with full authority to enter into an amicable settlement, to
submit to alternative modes of dispute resolution, and to enter into stipulations
or admissions of facts and of documents, and to act upon those matters provided
for in Sections 2, Rule 18 of the 1997 Rules of Civil Procedure, namely, the:

i. Possibility of an amicable settlement or of a submission to alternative


modes of dispute resolution;

ii. Simplification of the issues;

iii. Necessity or desirability of amendments to the pleadings;

iv. Possibility of obtaining stipulations or admissions of facts and of


documents to avoid unnecessary proof;

v. Limitation of the number of witnesses;

vi. Advisability of a preliminary reference of issues to a commissioner;

vii. Propriety of rendering judgment on the pleadings, or summary


judgment, or of dismissing the action should a valid ground therefor be
found to exist;

viii. Advisability or necessity of suspending the proceedings; and

ix. Other matters as may aid in the prompt disposition of the action.

6. To continue representing me in case of re-raffle of the above- entitled case;

7. To receive notices, orders, and other processes in relation to the above entitled
case , including all documents that are related or connected thereto ;

8. To make, indorse, accept, receive, sign, seal, execute, acknowledge, and deliver
deeds, assign-receipts and other such instruments in writing of whatever kind
and nature as may be necessary, convenient, or proper under the premises;

9. Perform all other acts and deeds necessary, related or incidental to the foregoing.

HEREBY GIVING and GRANTING unto my said Attorney-in-Fact full power


and authority to do and perform all and every act and thing whatsoever requisite and
necessary to be done in and about the premises, as fully, for all intents and purposes, as
I might or could lawfully do if personally present, with the power of substitution and
