A mother brings a child of 5 years old with grossly carious anterior teeth. Feeds child with
sweetened milk 4 times a day, supervised brushing 2 times a day with non-fluoridated
toothpaste.

13-year-old patient complain of pain on lower right back tooth for past 3 days. Pain is sharp,
continuous and throbbing, kept the child awake at night. Visited a few dentists for restoration
but child is uncooperative at times during treatment. Given a bottle of milk or juice each night
just before sleeping until he was 3 years old. Full set of primary dentition (20 teeth). All teeth
have caries except lower anteriors. There is a lingual fistula IRT 84 with pain on percussion
and grade 1 mobility.
Identify type of caries. Justify your answer. (5)
